Patient's Query

Hello doctor,

I am a 20-year-old male with a height of 6.1 feet and a weight around 132 pounds. I have been experiencing persistent issues related to my rectum or prostate for the past few years. This problem initially arose during sexual intercourse when I experienced discomfort at the point of ejaculation. I initially dismissed it as a minor issue but sought medical help from two different urologists, both of whom found no abnormalities upon examining my rectum and prostate. They prescribed medication to be taken with food, which has since become ineffective. I am in search of a permanent solution. I recently discovered that there is a history of colon cancer on my father's side of the family. However, a blood test performed by the doctors showed no apparent issues. To describe the pain, it has become more noticeable, particularly upon arousal. It originates from the rectum and extends upward to my stomach, just above the pubic area below the belly button. The rectal discomfort feels like intense aching and throbbing, almost like a sharp pinch, while the lower stomach pain feels similar to being punched. This pain has escalated over time, and I am concerned about its progression. The doctors inquired about urinary issues, such as leakage or difficulty urinating, which have indeed become problems occurring every other day. I am hopeful that you can offer assistance in resolving this situation, which has persisted for nearly two years and is steadily worsening. Kindly help.

Hi,

Welcome to icliniq.com.

I read your query and understood your concern. Typically, erections and ejaculation are pleasurable experiences that engage the brain. However, it appears that you are experiencing severe pain during these activities. Based on your history, it seems you have an adequate erection and ejaculatory function, indicating healthy vascular and efferent neurogenic components (nerves transmitting signals from the brain and spinal cord to organs). You have not mentioned any issues with constipation or changes in bowel habits, nor have you described urinary frequency or urgency. Additionally, your pain extends to both the rectum and the lower abdominal wall. These symptoms suggest involvement of multiple organs in the pain pathway, indicating primarily afferent neurologic involvement (nerves carrying information from organs to the brain and spinal cord). Analgesics may provide some relief but are unlikely to provide a cure. I suggest you consult a neurologist or pain specialist for a comprehensive evaluation and management of your condition. I hope this has helped you. Kindly follow up if you have more doubts.

Thank you.

Same symptoms don't mean you have the same problem. Consult a doctor now!
