Hi doctor,

I am a parent of a baby girl born a few days ago. We seek your advice regarding our challenges with her feeding and digestion. Our daughter started with Aptamil milk initially, but we switched to Novalac AC due to colic symptoms. Additionally, she developed a nappy rash, which we suspect was due to her inability to break down the milk proteins in the formula. After consultation, we transitioned her to a hypoallergenic formula, Nan LR. She consumes 120 to 150 ml of Nan LR milk every three hours. Her feeding is acceptable, and her stool is green-yellowish in color. Urination also seems normal, and her nappy rash appears to have decreased as we provide her with some cream.

Specific concerns are as follows:

  1. Fussiness and Sleep Difficulty: She remains fussy and has difficulty settling down to sleep.
  2. Quick Feeding and Reflux: She tends to gulp down her feed quickly, which leads to reflux issues. Despite our attempts to pace-feed her, she seems hungry yet simultaneously suffering from reflux.
  3. Regurgitation: After completing her feed, we burp her. However, she often ends up regurgitating the milk. Last night, during the feed, she regurgitated approximately 30 ml, and this morning she woke up early crying loudly with some milk pouring out from her nostrils.

She is also taking Infacol to assist with her colic symptoms.

  1. Is it common for a 1-month-old to experience these reflux issues?
  2. Do we need something in our feeding approach that might be causing these issues?
  3. Please provide some remedies or tips to alleviate these challenges.

I understand your concern.

Your baby is very much normal. Regurgitation issues are prevalent in babies up to three months of age. After that, they settle down. The thing we should be taking care of is weight gain. If the baby gains weight despite her regurgitation issues, there is nothing to worry about. For the feeding, I suggest you keep her feeding 120 ml every four hours for a week and gradually increase as per her appetite. Home remedies that can prevent regurgitation are burping after every feed. Use boiled water with fennel and carom seeds to prepare the formula milk at night before putting her to bed. During sleep, keep her head end raised. For babies with reflux problems, we initiate weaning, i.e., solid feed, a little earlier, around four months. Kindly let me know her birth weight, which will give an idea of whether she is gaining weight and how much. We have medication for regurgitation, but I prefer not to put the baby on medicines so early for a normal issue that will take some time to resolve.

Thank you for the reply.

In the attachment, I am attaching her complete weight progress. Regarding feeding, she does not last four hours; generally, she is hungry after two to three hours. We fed her 120 ml, although she did take 150 ml this morning as she was hungry. Regarding preparation, we prepare four scoops in advance but make sure they are consumed within 12 hours. Therefore, we found that this method satisfies her needs within five minutes. I want to add that only when she eats noisy breathing, like a high-pitched squeaking noise, is there.

I have seen your baby's weight gain record since birth, which is quite normal. The way you explained the feed preparation is also acceptable. It is fine to give her 150 ml if she is not satisfied with 120 ml feed. Just try the home remedy. I already told you to prepare the feed. As long as she is gaining weight despite regurgitation, it is not an issue. It will take time to settle down. Please take it as normal. Regarding the noise during the feed, we can wait, and it should settle down in two to three weeks.

Is adding fennel and carom seeds to the water safe, considering she is still one month old? And do we do this by placing the fennel & seeds in the kettle, adding water, and boiling all?
