Hello doctor,

I have had bursitis heel for the last seven months which is not healing in spite of ultrasound, healing creams, exercises, and anti-inflammation medicine. I suspect it is caused due to regular running. Please help.

I read your query and can understand your concern.

This is called Enthesitis and is due to inflammation of the plantar fascia. You need at least 10 days of anti-inflammatory medications.

1. Do an ESR (erythrocyte sedimentation rate), CRP (C-reactive protein), and if possible PDUs (power Doppler ultrasonography) of the heel.

2. Start taking the capsule Indocap (Indomethacin) SR 75 once a day along with Pantoprazole 40 mg in the morning.

You are aware of the anti-inflammatory medicine I have consumed. You are suggesting some more? I have attached some prescriptions, please have a look. I hope you had a look at the ultrasound scan report which was attached earlier.

The tests ESR (electrocyte sedimentation rate), and CRP (c-reactive protein) are inflammatory markers and PDUD is a power Doppler ultrasound that tells us about the vascularity of the inflamed area. I did see your ultrasound (attachment removed to protect patient identity), and yes you should start taking the medication that I mentioned earlier.

The hospital staff here are not able to understand PDUD. PDUD is a power Doppler ultrasound. Can you elaborate? I have completed the course on Indocap. However, ever since I stopped taking it, the pain has gone back to its original levels. Please help.

The labs here want to know if it should be arterial or venous. Also, my ESR done five months back indicates 11. I am yet to do a creatine test. I can share the last annual medical report done five months back which includes urine and others. Also, since stopping the Indocap, the pain has resumed.

What you should do is use soft shoes, preferably sports shoes, and not leather. Secondly, you can repeat a course of Indocap (Indomethacin) SR 75 along with Pantocid (Pantoprazole). Enthisitis is a recurring phenomenon, and PDUS (power Doppler ultrasound) is the only diagnostic test to confirm it. ESR (erythrocyte sedimentation rate) and CRP (c-reactive protein) are supplementary, and the good thing is that they are normal.

I want to ask the following questions -

1. How long should I use the anti-inflammatory medicine?

2. Power Doppler requires a prescription, is it possible for you to send me one?

3. What is a long-term solution? Oral medicine, physiotherapy, or surgery?

4. If calcification exists, how is it going to be dissolved or removed? Exercises or cortisone injections?

I saw your report which is normal. I suggest that two weeks of anti-inflammatory medicines are enough but should be taken with Pantoprazole to prevent gastritis. PDUS is a non-invasive test and does not need prescriptions. I would have sent you one but the format is not available.

I have started another course of anti-inflammatory medicine along with Pantoprazole, but this time there was dizziness for at least 2 hours. Please clarify if I should continue or stop. Also, I have taken it for two days after lunch.
