Hello doctor,

I have seen a dermatologist and urologist 10 to 15 times for the past few months. I was told I had a yeast infection and noticed physical changes in my penis and ejaculation and urination changes. The dermatologist I visited last week told me that it is not an infection or skin condition. After showing her pictures, she stated that the color changes could be normal, however, I have had my penis forever and know what it used to look and feel like. It may not be anything with the skin or an infection, but something has changed. I do not know if it is hormones or something else.

My urologist told me it was a mild case of balanitis and gave me some ointment that did nothing. My dermatologist gave me a cream that also did nothing. I went from lasting, however, long I wanted to in bed, to lasting 30 seconds on a good day ever since this happened. I also have a small bump on my shaft and two flat ones on my foreskin that were not there before the yeast infection. The dermatologist stated they were clogged pores and completely normal. The best way I can describe it is when I have an erection, there is instantly pre-ejaculate fluid. I have always had small amounts of it, but now it is non stop and starts as soon as I get an erection, it is so much that it is leaked through my underwear and pants at times.

And even after ejaculation and urination, it still keeps going for about an hour after ejaculation. I am saying that after ejaculation, there is still clear fluid leaking out for about an hour. Sometimes I can even feel it moving through my penis. Also, as I said, I cannot even last a minute in bed anymore ever since this happened. I was used to being able to last 30 minutes to an hour, however long I wanted. When I ejaculate now, my penis' top part will turn white, almost like releasing pressure or something. I have been tested for STDs and came back negative. I have had blood cell counts and urine culture done. Basically, I have had a lot of things done, but everything has come back normal. So no one has been able to explain the changes to my penis appearance or performance. All the things I put above did not happen before the yeast infection.

I have tried numbing cream, the squeeze method, and everything I can think of, and nothing is working. Also, sometimes I do feel a slight pain in my penis. It never lasts for more than a minute before it goes away and does not happen every day. Sometimes it will burn or hurt when I urinate, but most of the time, it does not. One time it did hurt enough to where I had to pull over on the side of the road when I was driving home.

At one point, I did have the bruising right at my urethra hole. But that lasted less than 24 hours. However, I did take a picture of it. I do not know what is causing this, and apparently, doctors cannot figure it out either, even telling me it looks normal. I can show what it looked like before and then what it looks like now. If it is not something with the skin, I can only think it is hormones or pressure somewhere. What concerns me is how much clear sticky fluid comes out after ejaculation. Even when I urinate after ejaculation (which should wash everything out), there is still sticky clear fluid coming out after that. I would understand if this was something that had been going on forever but being it just happened concerns me. It is very depressing, and I do not know what else I can do. I will put up as many pictures as I can.

I can summarize your concerns in two things which are, premature ejaculation, and post ejaculation long term discharge. Addressing the first issue, I would say if you tried coitus interruption and squeeze technique, etc., and it did not work, then you should start medication for that. You can take tablet Fluoxetine 20 mg twice daily. That will delay your ejaculation. Regarding post ejaculation dripping, it might be caused by early ejaculation or maybe due to retrograde ejaculation or over secretion of semen. I can give you medication, but I prefer to try one by one to start the medication. I gave you it might resolve your issue completely without the need to do anything more. Follow up after four weeks to see how it goes.

Is it normal for these changes to happen? I was confident in my bedroom. I just woke up and witnessed redness and a funny feeling in my penis that turned into all of this. It has completely ruined my confidence, and I do not know what is caused it.

Please do not worry about the skin color changes. There are variable and normal changes in the skin color of the penis as it is a part of body skin. But there are abnormal changes as well from the pictures you sent (attachment removed to protect patient identity). I can see that those changes are normal and do not need to worry about.
