Hello doctor,

I am 18 years old and suffering from a ringworm infection in my genital area. I cannot go to consult a doctor. I have itching and rashes around my genital area. It has spread over a large area. I request you to tell me about medicine and cream so I can get rid of it. I have severe itching. I can easily recognize that it is ringworm infection. I used some antifungal ointments, but they did not work. Please help me overcome the ring-like rashes due to the infection.

As per the history and image provided (attachments removed to protect the patient's identity), it appears as Tinea cruris which is a ringworm infection mostly appearing in the folds, including genital and perianal areas. Fungal infections appear on folds due to the stagnation of moisture, sweat, and lack of ventilation in that area well. They may also spread from person to person by sharing infected belongings and reduced immunity. They usually present with a round to well-circumscribed areas of silvery white scales and intense itching, which may be uncomfortable. Therefore, I suggest you do the following. Use Ketoconazole soap for bathing. Lather with water and apply it to the affected areas. Leave it for three minutes and wash it off. The unaffected areas of your skin can be washed with regular soap. Terbinafine cream twice can be applied after bathing and towel drying the areas well. The cream is to be applied to the affected areas only. Terbinafine dusting powder must be applied to the affected areas to avoid stagnation of sweat and moisture. Take the capsule Itraconazole 200 mg once daily at night after food for four weeks. Take the tablet Cetirizine 10 mg one hour before going to bed for two weeks. This medication may cause sedation, so do not drive after taking it. It helps in getting rid of the itching associated with ringworm. Avoid moisture in the folds and genital areas. Clean and dry the areas well after bathing. Wear loose-fitting clothes and dry the area well under a fan to avoid moisture stagnation. Do not share belongings from infected individuals. Consult a specialist doctor, discuss with them, and take medications with their consent. Ringworm infections usually take time to completely go away. It may appear on and off in case of moisture stagnation, sweating, and low immunity. Consume enough water daily to keep yourself hydrated as well. Consistency is key when it comes to the treatment of ringworm infections; therefore, be patient and keep going.
