Hi doctor,

I have been diagnosed with severe scabies in my tissues, but my treatments have not been successful. Tablet Permethrin and tablet Ivermectin for two days did not help, and I think I need more days in a row. I wonder if Ivermectin 1% topical lotion would work for my body?. That was prescribed for me now. Is it safe to take both Ivermectin oral pills same time as Ivermectin topical lotion? or is it better to do them separately, like three days of the tablet, then rest with lotion for a week? How is each of them being processed in my body? Please help.

I understand your concern. I suggest you not use both topical and oral Ivermectin simultaneously. You can use only oral Ivermectin because oral therapy kills germs of scabies completely. I suggest you take tablet Ivermectin for three days again. I suggest applying Physiogel lotion (two parts) and Betamethasone lotion (one part) combination on your body once daily for two weeks. I suggest tablet Cetrix 10 mg (Paracetamol 500 mg, Phenylpropanolamine 25 mg, and Cetirizine 5 mg) at night for two weeks. Hope this helps.

Is it possible to take the Ivermectin oral for five days in a row? Is it safe? Two days in a row did not help, and I am almost sure that three would not be enough. I have them on my scalp and find it hard to get rid of them. Any suggestion of treatment for that? But if I want to try oral and topical Ivermectin, is it safe to do so? What do you mean by two parts and one part? Do I mix them (Physiogel lotion and Betamethasone lotion) at the same time? Please help.

I understand your concern. You can take oral Ivermectin for three consecutive days then repeat it after ten days. Combining topical and oral Ivermectin can lead to toxicity. And yes, you should mix Physiogel lotion with Betamethasone lotion at the time of application. Two parts means quantity of Physiogel lotion should be double the amount of Betamethasone lotion. Hope this helps.
