Hi doctor,

Since I can remember, my scrotum has always been extremely dry. My parents used to say it was eczema. I used Hydrocortisone cream, but I have stopped using it now because I want to get it treated naturally. Since I stopped the cream, the itching has worsened, and the dryness is severe. My scrotum cracks and bleeds. In addition, it is now affecting the side of my penis shaft that is lying down. Warm or hot water wash soothens it. Sometimes I cannot stop scratching until it starts bleeding. This scratching is severe at night. What can I do to cure it? Is this eczema or a lifelong jock itch? Please help me.

I went through your query and understood your concern. I have viewed your shared images (attachment removed to protect the patient's identity). It is scrotal eczematous dermatitis, and it seems that it is infected with dried crusting around it. I suggest you to follow a few instructions mentioned below. Take an oral antihistamine like tablet Cetirizine 10 mg, for symptomatic relief from itching. Also, take an oral antibiotic, either tablet Amoxicillin or tablet Cephalexin for a week to help resolve the infection. Apply a moderately potent topical steroid like Mometasone Furoate 0.1 % cream Or Fluticasone Propionate 0.05 % cream twice daily for two weeks. However, since topical steroids cannot be used for a long time, you may use a topical Calcineurin inhibitor like Tacrolimus ointment for a slightly longer duration to prevent relapse. Kindly consult a specialist, talk with them and take the medications with their consent. I hope this was helpful. Take care.
