Hello doctor,

I have severe anxiety, depression, a fast heartbeat, sweating, IBS, and diarrhea. I have had a nervous weakness for the last 15 years. I have consulted many psychiatrists, but none of the medications suited me. I am currently taking tablets Nexito forte and tablet Pronol, and tablet Pristiq 50 mg (Desvenlafaxine) once daily at night. I have been taking this medicine for the last two years with some relief, but the tablet Pristiq causes excessive sweating in the daytime without much control over my IBS and diarrhea problem. My doctor changed my medications twenty days ago from Pristiq to Libotryp DS (Amitriptyline 25 plus) at night, and the other medicines are the same. Libotryp DS improved my IBS and diarrhea problem but created more sedation in the daytime. Now I am very much confused about what to do and how I can continue with my daily activity. I do a job and cannot take daily leave due to my anxiety and depression problems. Kindly help.

I understand the kind of distress you are currently going through. Since you mentioned that it had been such a long time, almost fifteen years, you finally got the medication that you are quite comfortable with and adjusted to it. So your anxiety and IBS (irritable bowel syndrome) symptoms are better now. The only problem here is your sedation, probably because of the tablet Nexito forte (Clonazepam and Escitalopram) 0.5 mg. So I suggest you take tablet Escitalopram and tablet Clonazepam 0.25 mg separately instead of their combination for the next two weeks. You can stop taking Clonazepam after two weeks to solve your sleep issue. Consult your specialist doctor, discuss with them and medications with their consent.

My main confusion is whether I should take the tablet Pristiq 50 mg or Tryptomer 25 mg at night. The doctor prescribed me Libotryp DS, which causes excess sedation in the morning. One of the doctors also suggested taking plain Amitriptyline 25 mg at night instead of Libotryp DS and Pristiq. Kindly suggest which medicine I should take.

I suggest you continue taking tablet Libotryp DS (Amitriptyline and Chlordiazepoxide) or Tryptomer (Amitriptyline) 25 mg, Nexito (Escitalopram) 10 mg, and Clonazepam 0.25 mg (which you can stop taking after two weeks). Consult your specialist doctor, discuss with them and take medications with their consent.
