Hello doctor,

I did not have my mensuration cycle for about three months. I also think the semen of my husband would have fallen on to my vagina accidentally. I am worried if I am pregnant and about taking an I-pill. Kindly help.

I can understand your concern.

Based on the description given by you,

Semen or the pre-seminal fluid (the clear fluid that comes out before semen - which is rich in active healthy sperm) even if it comes in contact with the external genitals or even on the thigh, the sperms can swim up by chemical attraction towards the egg and cause pregnancy. As you do not have periods, we cannot be sure if you have ovulated or not. So if you are sure not to have a pregnancy, have the emergency pill immediately.

Evaluation is needed if there are no periods at this young age. I suggest you follow the below-mentioned instructions:

  • Get a urine pregnancy done to rule out pregnancy. Sometimes ovulation can occur even without periods. I have had an experience where the patient came in pain abdomen and a big mass in their abdomen. It was actually pregnancy with labor pain.
  • Get a thyroid profile and serum prolactin on an empty stomach at 6:00 am to 09:00 am. This will need correction for regular periods
  • Hope you are not using any other hormones or medicines
  • Sudden decrease or increase in weight or recent excessive stress like mental or physical strain, depression, or anxiety.
  • Your weight is not in favor, but have a scan to confirm that uterine size is normal and that you do not have PCOD.
  • If all the above tests are normal, have a serum AMH ( Anti-Mullerian Hormone) - this is a guide of the number of eggs left in the ovary.
  • After having an emergency pill you will have periods within seven to ten days. From the first day of the bleeding have any of the birth control pills for one cycle, that is 21 days. This will regularize your hormone balance and periods. From next month your periods will be normal.
