Hello doctor,

I am a 28-year-old male, and I have two questions that I need your help with; firstly, the head of my penis, especially the tip, is really sensitive, and I get an extreme ticklish sensation when I touch it. It happens just after the lubricating fluids come out before ejaculating during masturbation. It gets worse after ejaculation that I cannot even touch the penis tip and find it hard to finish the blowjob since it tickles so much on the tip. There is no pain or irritation, the physical appearance is normal, and I do not have any discomfort during urination. I masturbate almost once daily and have been facing this problem for three years. I do not have any problem with masturbation as long as I do not touch the tip of the penis after ejaculation. Also, I clean the area with a cloth and often wash with water and soap. Why is this happening? And what will be the best treatment?

Secondly, I am concerned if I have performance anxiety during sex as I find it difficult to achieve an erection when I try having sex. In contrast, it happens without any trouble when I masturbate. Also, I find it hard to have an ejaculation when I have sex. I have tried having sex with different girls, and the same thing happened. I do not have a history of depression or anxiety, if that might be a concern. How do I cope with this? Can this be due to frequent masturbation or performance anxiety? Do I require any medication or tests?

Regarding your first question, an extremely sensitive penis tip can be due to balanitis (inflammation of the penis head and foreskin). In this condition, the tip will be slightly red and very sensitive and require a urologist's physical examination to confirm the diagnosis. If balanitis is confirmed, using an antibacterial ointment and mild steroid will help. Occasionally, extremely sensitive glans can also be present normally in a few individuals. In such cases, using a condom during intercourse will help prevent extreme sensitivity, and if the problem persists, the use of local anesthetic cream containing Lignocaine will help. Also, having a normal erection during masturbation and not during sex is generally anxiety-related. As you get a normal erection during masturbation, your erection mechanism is fine. Also, yes, frequent masturbation can cause a delay in ejaculation. So, please try not to masturbate for at least 72 hours prior to having sex for a proper ejaculation.
