Patient's Query

Hello doctor,

I have been taking Sertraline Hydrochloride for an extended period. Over the past year, I engaged in sexual activity multiple times (approximately 20 instances). However, I have been experiencing a diminished sensation in my penis during intercourse, perceiving only slight warmth from my partner's vagina. I have confirmed that I am HIV negative. My doctor conducted tests for Luteinizing hormone (LH), testosterone, Follicle-stimulating hormone (FSH), prolactin, vitamin B12, and serum folate, all of which yielded normal results. I do not encounter difficulties achieving or maintaining an erection before and during intercourse. I am a 29-year-old male in good health with no inhibitions or shyness contributing to this issue. I am unable to discontinue sertraline. I kindly seek your advice on this matter.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern. Firstly, I would like to inform you that Sertraline Hydrochloride tablet might potentially be linked to your concern, although before drawing any conclusions, I need to gather more information such as, mentioned below:

  1. How was your first sexual experience?
  2. Do you encounter the same issue during masturbation?
  3. Has the problem worsened or remained stable over the past year?
  4. How long have you been taking sertraline?
  5. How long have you been sexually active, whether through intercourse or masturbation?
  6. Are there any underlying neurological conditions?

There are alternative options similar to Sertraline Hydrochloride that might not have the same sensation-related side effects. After a comprehensive assessment of your sertraline requirement, we can make an informed decision. Alternatively, you could explore the option of reducing your Sertraline Hydrochloride dosage by around 20 % under medical guidance. I hope this has helped you. Kindly follow up if you have more doubts.

Thank you.

Patient's Query

Hello doctor,

Thanks for the prompt reply. Below are the answers to your questions:

  1. During my first time intercourse, I could feel absolutely nothing.
  2. when Sertraline was reduced from 100 % to 25 % then I could feel amazing sensation during masturbation but I never had sex during that time. Sertraline had to be increased back to 100 % as reduced dose had very bad effect on my mind.
  3. The symptoms progressively improved but very marginally.
  4. I am on Sertraline for almost 13 years.
  5. I have been masturbating since the age of 13. I have been sexually active for more than one year now.
  6. No, I have no known neurological problem.

As mentioned, I cannot reduce the dosage of Sertraline Hydrochloride as it has very bad effect on my mind. So it is first important for me to have a stable mind. Could you suggest on, what to do and also, if there is anything I can take just before intercourse?

Hello,

Welcome to icliniq.com.

I read your query and understood your concern. Based on the information provided, it is apparent that sertraline is likely responsible for the loss of sensation during sexual activity. I suggest you can follow the below mentioned instructions as potential solutions:

  1. Since your sexual activity is intermittent, you could try skipping a dose if you anticipate sexual activity the next day. Sertraline's half-life of around 26 hours means that occasionally skipping a dose in a month is unlikely to impact its efficacy.
  2. While this method has a slight chance of not working for everyone, if issues persist after a couple of attempts, consider skipping the dose on the day of sexual activity and resume your sertraline afterward.
  3. Other medications like Fluoxetine, Escitalopram, Citalopram, Fluvoxamine, and Paroxetine share similar mechanisms of action but differ in side effects. With your psychiatrist's guidance, you might switch to one of these alternatives, using a cross-tapering approach to maintain efficacy.
  4. Your ability to feel some warmth indicates nerve functionality in your penis, suggesting that sensation is not entirely impaired.
  5. Enhance foreplay during sexual activity, focusing on increased lubrication and sensation. Foreplay's importance often outweighs intercourse itself.
  6. If you use condoms, the type you choose could affect sensation. Choose ultra-thin varieties if applicable.

These suggestions are my personal opinions and might be beneficial. Currently, no official recommendations exist for this specific issue. I hope this has helped you. Kindly follow up if you have more doubts.

Thank you.
