Q. I have heart palpitations, heartburn and shortness of breath. What is happening with me?

I notice some palpitations in my heart. I have heartburn at random times. I have shortness of breath. I start to have a cough when I lie down at night and I tend to choke easily if I lie flat on my back. I have done some tests. The results came back normal and clear. I have done a chest X-ray and I have described my findings. My cardiac size and contour are within normal limits. My hila regions appear normal. My pulmonary vasculature appears normal. Both my lungs are clear. Both domes of the diaphragm are normal. Soft tissues and ribcage are normal. There is no significant abnormality detected. I did an ECG and it was normal. I was advised to do echocardiography. Do I need to do echocardiography at this point? As my ECG was normal, do I need to do echocardiography?

The possible causes of your symptoms are acid reflux, nocturnal asthmatic attacks, orthopnea seen in heart failure, and obstructive sleep apnea. You need to do TMT (treadmill test), echocardiogram, polysomnography, serum pro-BNP (pro-B type natriuretic peptide), and 24-hour esophageal pH monitoring. You need to do these tests to know your possible cause. Avoid drinking cool drinks and ice creams. Avoid smoking and alcohol consumption. You can take tablet Pantoprazole before breakfast for a week. You can take tablet Levocetirizine once a day at night. Have a review with your reports.
