Patient's Query

Hello doctor,

I am currently 46 years old and have recently begun experiencing changes in my sleep pattern. I want to provide some context regarding my history of substance use. In the past, I struggled with addiction to cannabis, alcohol, and various medications, including Nitravet, Diazepam, and eventually, tranquilizers such as Escitalopram, Tricalcium, and even my favored choice, Librium 10 mg.

At present, my diagnosis includes the following medication regimen:

Morning:

  1. Two tablets of sodium valproate (200 mg each).

  2. Glycomet 500 mg.

  3. Either Fourts-B or B-complex supplement (like Neurobion Forte).

Afternoon:

  1. One tablet of sodium valproate (200 mg).

  2. Dynaglipt for diabetes management.

Night:

  1. Two tablets of sodium valproate (200 mg each).

  2. 100 mg of Quetiapine (Seroquel).

  3. 10 mg of Nitrazepam.

  4. 500 mg of Glycomet.

In terms of physical activity, I engage in a 20-minute walk both in the early morning and in the evening hours, along with warm-up exercises in the morning. My height is 5 feet and 1 inch and my weight is 125.7 pounds. My primary concern is my sleep quality. Recently, I have been able to sleep at 10 p.m. and wake up at 5 a.m. without any interruptions. This has been consistent for the past three days, which is a change from my usual sleep pattern. I am reaching out for advice on managing my sleep disturbances, considering my medical history and current medication regimen. Your insights and recommendations would be greatly appreciated. Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

Thank you for your query.

You have a history of polysubstance use disorder, involving the usage of multiple substances in the past. To address your current situation, it is important to follow the prescribed medication regimen provided to you. Additionally, adopting a healthy lifestyle can help manage your sleep-related issues. Here are some recommendations:

  1. You need to fix your time for sleeping, which means that you must fix the time of going to bed and waking up in the morning.

  2. You must not do any other activities in bed.

  3. You must not just lie down on the bed without any reason. The bed is just meant for sleeping.

  4. Wear loose and comfortable clothes while sleeping.

  5. Take light food at night or have your dinner at least three hours before sleep.

  6. You can take a maximum of 20 minutes of nap in a day.

  7. Do not use a mobile, TV, or laptop for at least half an hour before going to sleep.

  8. Try to maintain an adequate temperature in your room (neither hot nor cold).

  9. Have a proper bed and pillow.

  10. Make sure there is no noise or disturbance at night.

  11. You can drink warm milk with turmeric before going to sleep.

  12. If you still have issues with sleep, then you can take medication under the guidance and prescription of your psychiatrist.

I hope my advice will be helpful. Feel free to ask if you have any further queries.

Thank you.
