Should an enlarged spleen be a matter of concern?

Hello doctor,

I am a 32-years- old male, 5.61 feet, and 178 lbs. I have had an enlarged spleen of 5.51 inches to 5.91 inches for many years.

No other symptoms, and every six months I get my blood work done with normal results. Should I pursue this more? My pathologist says it could be my normal-sized spleen.

I read your query and understand your concern. An enlarged spleen is seen in liver diseases, hemolytic anemia, parasitic infestation, especially malaria, autoimmune disorders, and rheumatological diseases. An enlarged spleen is not normal at all; it can cause symptoms, sooner or later.

A 5.51-inch to 5.91-inch spleen is considerably enlarged. I have never seen a normal spleen of this size in my life. I suggest getting a hemoglobin electrophoresis, erythrocyte sedimentation rate, CRP (C-reactive protein), LDH (lactate dehydrogenase), ALT (alanine transaminase), and creatinine levels for further evaluation.

Hello doctor,

Thank you for your response.

I did the recommended tests, and my GP says they are normal.

All of the below are within normal ranges:

1. ESR: 2 mm/h

2. LDH: 316 units/L

3. ALT: 12.6 U/L

4. AST: 17.9 U/L

5. Creatinine: 0.97 mg/dL

6. Total bilirubin levels: 1.2 mg/dL

Good to know that you have already been tested and the results are in range. However, bilirubin total is at the upper limit, so keep monitoring bilirubin on a monthly basis.

Get a course of anthelmintic and anti-malaria on an empirical or prophylactic basis.
