How to treat constipation with abdominal pain in a child?

Hello doctor,

My daughter, who is 2 years and 7 months is complaining of mild "stomach pain" around the navel area for the past two weeks. Because of the current pandemic situation, we are somewhat hesitant about taking her physically to a doctor. The pain, which comes on and off, comes 3 to 4 times a day and lasts for a few seconds to a few minutes. Though she does not become restless when it occurs, she does lie down on her stomach for a while or starts stroking it with her palm. Incidentally, of late, she is very constipated and passes hard stools only once in the day or, at times, skips pooping for the entire day. She drinks very little water (despite our insistence) and refuses to take any fruit other than a banana and sweet lime (in juice form). Had deworming medicines about eight months ago. Please advise.

Syrup Zentel (Albendazole) 10 ml single one dose at night (to deworm). Syrup Rantac (Ranitidine) 5 ml twice a day before food (for gastritis pain). Syrup Cyclopam (Paracetamol and Dicyclomine) 5 ml when required for acute pain abdomen.

The Probable causes

Constipation.

Investigations to be done

USG abdomen.

Differential diagnosis

Constipation with pain abdomen under evaluation.

Treatment plan

Increase water intake to 0.39 gallons per day. Increase fruits and vegetables intake. Avoid refined products like biscuits, cake, bread, noodles, pasta, chocolate etc.

Regarding follow up

If no improvement, follow-up with USG abdomen.
