Hi doctor,

I am 27-year-old. My height is 68 inch and I weigh around 235 pounds. I am currently a manager at a grocery store. I typically work ten hours a day each week. I have been working at this job for about 11 years now. I am at the point now though where each day is taking a toll on me. My work schedule has no consistency. One day I will work from 4am to 3pm then the next shift is 12pm to 11am. Everyday I feel tired. I rely heavily on energy drinks. I have lost interest in hobbies or doing anything on my days off. I feel like I need to just rest or relax or watch television. I am having trouble sleeping. I am resorting to drinking almost everyday out of boredom and to help me sleep. My friends and girlfriend make comments all the time about how grouchy I am and how easily irritated I am. She will want to hang out when I get off work but most of time I just want to relax or sleep. My body does not ever really feel good anymore. Usually tired and sore from work. I was wondering if I could get some advice on what I should do or any medications I should seek to help me.

I am here to answer all your questions.

Thank you for providing a good history. I would suggest starting off with thinking about simple things like vitamin D deficiency. I think you are mostly indoors from the type of job you do. Therefore, most likely the lack of energy is coming from there as well. Vitamin D deficiency can present as depression, lack of interest and energy. If you have a doctor I would suggest getting tested for it. If you have a family history of diabetes or have gained weight then routine checkup diabetes and thyroid issues would be the second most important one. If all these four elements are fine (vitamin D, vitamin B12, diabetes, and thyroid), then I would think of depression and, yes, would consider therapy first and then medication.

Physical activity - Exercise three times in a week (I know you would say I have no time). Once you start doing it, believe me, you can do. It is an excellent natural source of antidepressant. Try it, you will not be disappointed.
