Hi doctor,
My wife is 23 weeks pregnant. She has been diagnosed with an 3.1 inches submucosal fibroid. She is in agony everyday with pain. I know more likely she will need a C-section eventually for safer delivery. But should I do a CA-125 test for peace of mind to rule out ovarian cancer.

I understand your concern.
1. CA125 is not relevant in pregnancy and it does not help in the management of a submucous fibroid.
2. After delivery the fibroid can be removed at the time of C-section by an expert doctor.
3. Hysterectomy is not needed for a fibroid of this kind.
4. Yes, this fibroid is a matter of concern. There is a likelihood of - premature delivery, abruption, fetal growth restriction, low birth weight, bleeding, degeneration of the fibroid.
At this stage all you can do is to watch it. Take simple painkillers like Paracetamol off and on for pain, take complete bed rest and avoid intercourse. The only thing that worries me here is that is it a fibroid.
Was it diagnosed on ultrasound or MRI? Pregnancy itself can impact the visualization of masses and rarely there is a possibility that it could be an ovarian mass or a subserosal fibroid. If her pain does not settle, she may have to go for an MRI. Please start looking for a good neonatal ICU unit for the baby incase delivery needs to be done before time.
In very good hands 28 to 32 weeks baby also has a good prognosis.
