Hi doctor,

I have a 16-year-old crown at the top left side of my mouth, and it has been chipping out for the past three days. I have already fixed an appointment with my dentist for this. But, about ten hours ago, I was talking, and it felt like something went down my throat. It felt like I swallowed a bug or something. Then I wondered whether it was the chip that had come off the crown. I do not know how large it was, but I do not think it would have been very big because the crack in the crown does not look a lot larger than it did earlier today. For a few hours, it felt like something was stuck in the back of my throat, but that feeling is gone now. A few hours ago, I started to have a stomach ache, loose stools (I have not noticed any blood in the stool), nausea, and a gas problem. Is it possible that the tooth chip could be causing a problem in my digestive tract, or could it be coincidental? The gas problem and stomach ache have settled down a little now, and I have not had a bowel movement for the past three hours. My stomach is still upset, though. I am worried. Should I consult a doctor? Also, I usually take Pepto-Bismol when I have diarrhea or stomach upset. But I am afraid to take it because it may turn my stool black. Currently, I am taking Bisoprolol, Wellbutrin, Mirvala, and Dexilant. Kindly help.

Thanks for writing in, and I understand your concern.

Zirconia crown cannot cause any bleeding because it is completely biocompatible and is easily excreted from the body. But metal crown can take some more time to get out, but it will be out during normal bowel motion. Already you have taken laxatives and acidity drugs. So please do not worry. I suggest you take regular food and drink plenty of fluids.

Thanks for the reply. You made me feel so much better. I have bad health anxiety and have been worried for hours. I tried to contact my dentist, but I have not heard back since it is after hours. I am sure my crown was porcelain and not metal, so maybe that is why it is chipping. I did not know if it would cause digestive problems or bleeding.

So it is fine for me to use Pepto-Bismol for my stomach problem? I was just worried about using it as it turns stool black.

I suggest you not take any further medication as you have already taken acidity and stomach ache medications. I recommend you to take Lactulose solution if you have a severe stomach ache. I also suggest drinking lots of fluids.

My dentist called me today and agreed with your advice that my stomach upset is not likely a result of swallowing a chip of my crown. I hope that I can clear your discomfort. See, it is not canker sores. But it is a traumatic ulcer caused due to either hot food intake or forceful frictional forces given after swallowing. Do not worry. I am prescribing some medications. It will get healed. Dologel (Choline salicylate and Lidocaine), or Ora heal (Choline salicylate and Lidocaine), or Orasole (Benzocaine) can be used. If the pain is severe, use it twice a day for five days. It is a topical analgesic gel. Multivitamins whatsoever suit you, take them once a day for fifteen days. Zincostar (multivitamins) can be taken. Please confirm whether you are not taking any other medications like anticoagulants or corticosteroids, or antidepressants. Avoid eating hot food on that side. Avoid eating tea on that side. Please update me after five days if no improvement is there, and please confirm if you are allergic to any of the above medications because that will trigger another reaction to the palate. Regards.

Thank you so much. So, you do not believe this to be infected? It is an ulcer from the injury to the roof of my mouth. I bought an analgesic gel today. I will attach a photo to see if I bought the right one. I am unsure if I should take an antibiotic, and my dentist told me that I do not have an infection. I have a bottle of Penicillin 300 mg prescribed for possible strep throat, which I did not end up using. Should I start using it? Would it even help if I do have an infection? I am taking Bisoprolol, Wellbutrin, Dexilant, and Marvelon. I do not have any allergies to medications. Thank you.

Thanks for your reply. It is infected but not upto that much that you will need antibiotics. So do not take antibiotics for it. The gel you have taken is fine, and instead of my prescribed gels, you can take it upto two times for five days. Except Wellbutrin, all medications do not pose any problem. But this medication can interfere with Zincostar, so do not take it. However, multivitamins you can take have so many interactions. Lastly, please do not worry. Just follow my instructions, and you will be alright within four to five days. Regards.

Thank you so much. I am so afraid of infections. I will not take Penicillin if you think that it is not necessary. I am not familiar with Zincostar. Does it come under another name here? Is it a pain killer or mouth rinse, such as Listerine? Should I gargle with saltwater? Also, do you think this could swell and affect my swallowing or breathing, or do you think this is as large as it will get infected? I am sorry for all the questions. Unfortunately, I have health anxiety. I do appreciate you helping to answer my questions and calm my fears. Thank you.

No worries. I am here to decrease your discomfort. Zincostar is an antioxidant. But it interferes with some medications. So please avoid it. Please do not do any rinsing with saltwater or any mouth wash. Instead, if available, use Betadine (5 percent) solution mixed with water for rinsing twice a day. It will help in faster healing. Just go by instructions like avoiding any hot food intake or coffee or tea on that side for atleast four to five days. It will heal itself. Nothing more is required. Betadine is popular by the name, Povidone. Just put two to three drops in a glass of lukewarm water and rinse with that. Regards.
