I have a deep white matter shown on my MRI. I have some spots on my left lobe. The doctor says me that it is caused due to headache and it is not MS. What it is?

These types of spots are known as nonspecific white matter hyperintensities. They are seen in patients with advancing age, in hypertensive conditions, in smokers, and in patients with migraine. Your doctor has told you the right diagnosis. It can be due to migraine headaches.

I am 42 years old and I have headaches every day. It disturbs me a lot and I cannot do my work. I have tried taking many medications. Is there anything that can help me?

First of all, you need to leave the MRI (magnetic resonance imaging) finding as it is nonspecific. I need to know the cause of the headache and you need to provide a complete history of it. Please mention the onset of headache with duration, type of headache whether it is throbbing or dull aching type, any features associated with it such as nausea, vomiting, phonophobia, about your sleep, and any other relieving factors. Please mention the medications taken for it and the duration of taking it.

I have headaches every day. In the past few years, it became more frequent. I took Ibuprofen and Tylenol. Both of these medicines did not help me. Now, I have a tension-type headache for the past seven months. Sometimes, it is on the left side. It affects my vision and I have muscle and joint pain especially on hands. I have deep chills on and off and it seems to be flu. I got tested for COVID-19 and the result was negative. I wet my bed during sleeping. I did it four times this month. I am unable to urinate at other times. I have a history of WPW and it was ablated. Before seven years, I had atrial fibrillation and it was ablated before two years. Now I have ectopic beats with PVC and SVT. I am taking Diltiazem 220 mg for a year, tablet Perindopril 6 mg for blood pressure for five months, Escitalopram 20 mg for five years, and tablet Remeron 15 mg for four months.

You are suffering from depression, tension-type headache, WBW syndrome (Wolff-Parkinson-white-syndrome) post-ablation, and urinary incontinence. I suggest you take tablet Topamac (Topiramate) 25 mg twice a day for three months along with other medicines and take tablet Naproxen 500 mg for headache. Consult a urologist for urinary incontinence. Have a review after three months.
