Hello doctor,

I got a hand job from a friend. She spat on it a few times for lubrication. A few days later, my left testicle feels a bit tender, it is very mild, though, and I am urinating more frequently. I also feel a slight pressure on the head of my penis. In the morning it is fine, I do not have a problem. I think that I am flexing that area which is causing blood to rush in and causing tension. I have undergone a urine test. It was negative. Is there a high chance of an STD? If not, what else could be causing it? There is no discharge or anything.

Thanks for writing in. I understand your concern. Urine analysis will be negative because the genital system and urinary system are separate. I think you have an inflammation of your prostate. To check it, you need to undergo a semen culture test rather than a urine test. Based on your test result, I can suggest antibiotics. Saliva contains many germs which can cause infection of the prostate. Please do semen culture and do follow-up for further assessment and treatment. If you have any more queries, please let me know.

I will follow your instructions, doctor. How do you get prostate inflammation? What causes it? I am currently on Azithromycin for pneumonia. Will that work?

You will get prostate inflammation from the saliva or the spit used in the hand job. And as I mentioned, saliva contains many bacteria which can cause infection. Azithromycin might work, but as I do not have the culture test reports, I am not sure. Antibiotics used for prostate inflammation will take four to six weeks to cure prostatitis.
