Patient's Query
Hello doctor,
I just came to know that I am a thalassemia carrier. I have never had any problems in the past. My RBC count is higher than usual and my platelets count is low. Is it serious?
Kindly help.

Being a thalassemia carrier, the main risk is of transferrin thalassemia to the next generation, if your spouse is a thalassemia trait. So genetic testing of your partner is essential. In thalassemia, because the RBCs are smaller, the count is usually high and this is normal. Low platelet counts are difficult to explain as thalassemia does not affect platelets. Associated vitamin B12, folic acid deficiency, or other causes may require evaluation. The count at present is adequate and any further fall can be a very serious issue. The platelet thing requires specialist evaluation. Need to check for serum vitamin B12 and folic acid levels and if necessary bone marrow aspiration biopsy. Hope there was no fever in the recent past. Consult a specialist once and regarding thalassemia, genetic testing counseling of your partner is essential.
I hope this has helped you. Thank you.
