What is the treatment available for throat ulcers?

Hi doctor,

I had a bad throat recently, and I took Doxcef 200 for five days from a previous prescription, thinking it would help. However, now I have a throat ulcer (I guess), as my throat hurts when I gulp. It is predominantly on the right side.

It happened once last year, and based on that prescription, I am doing Betadine gargle and taking a Bevon capsule once a day. I am not taking Rebagen although the doctor prescribed it. There is no improvement and moreover, I feel that my stomach is gassy all the time with belching. Please let me know how I can take care of this.

I read your query and understand your concern.

I would say that you made a mistake by taking the medicines from the previous prescription. Most likely, you have caused unnecessary gastritis (inflammation in the stomach lining) due to the proper dosage of antibiotics pertaining to the current problem. Most of the throat problems like soreness and cold are due to viral causes and seasonal changes. On top of those weather changes, allergies and pollution add to the soreness. One does not need a strong antibiotic like Doxcef (Cefpodoxime proxetil and Clavulanic acid) 200 every time this occurs.

Now that you have taken a course of strong antibiotics, it is likely that the friendly bacteria in your stomach have been destroyed, and resistant bacteria have colonized causing vitamin deficiencies. These cause the current problems you face. You need to stop the gargles.

If possible take a picture of your throat with an open mouth showing, details of the back of the throat with tonsils, and send it. I would like to check if there is post-nasal drip (a condition where when more mucus than normal gathers and drips down the back of your throat), granular pharyngitis (a chronic inflammatory and/or irritative process, with no infectious component, of the pharyngeal mucosa), allergic mucin, or tonsillitis.

Next, you need to follow some dietary changes until you get better. You would need a proton pump inhibitor taken on an empty stomach for appropriate dosage as per your weight. Depending on the symptoms you may need a prokinetic drug like Domperidone. To help you better I want more details about your problems and habits.

1. Does gas formation occur after diet or is it when your stomach is empty, that is, between meals?

2. Do you smoke, drink, or chew tobacco or betel leaves?

3. Do you eat spicy and oily food?

4. Do you include vegetables and fruits in your diet?

5. How are your bowel habits? Is it normal, loose, or constipated?

6. Do you have a burning sensation in the mouth?

Thank you for your reply.

I understand self-medication is very dangerous. But, I had to take it to continue my job. I will not do it again. I used to smoke, but stopped since I had a bad throat. I have not taken alcohol in a week now. Please advise on what needs to be done.

I wanted answers to all your questions so that I can guide you in a good way. Nevertheless, I shall suggest some measures to you. Follow these, and do revert back to us in two days, and let us know how you feel.

1. Avoid all spicy and oily foods, alcohol, and smoke till you start feeling fine.

2. You need to take Pantocid DSR capsules (Domperidone 30 mg and Pantoprazole 40 mg) twice daily, half an hour before meals. You would need this for a minimum of two weeks, thereafter we can change the medicine to another depending on the progress.

3. You need a Montair LC tablet (Levocetirizine 5 mg and Montelukast 10 mg) to be taken once in the evening daily for two weeks. Consult your doctor and take the medicines accordingly.

4. Do not do gargles. But take steam inhalation. Avoid talking for long durations.

5. Wear a mask when you venture outside.

Review with us in two days with a fresh photograph, and we shall guide you on how to go about it after two days.

If there is no improvement, I would start you on a Macrolide.
