Hello doctor,

My mother has a bad throat for a month now. I believe it is because of excessive yelling all the time due to some reasons. She went to an ENT three weeks before and that did not help even a bit and there were other side effects of gas, worsening of piles, and constipation. She has pain in her throat while even drinking water. Earlier she used to have these pain too because of excessive yelling but that got healed itself within a day or maximum two days but this time it has been a month. Also, the ENT we took her to said that it is an infection. And also my mom just told me that she experiences some very tiny ulcer kind of things inside the throat with her tongue which she cannot see in the mirror though. Please guide.

With the symptoms and the pictures you have sent (attachment removed to protect patient identity), it appears your mother has some laryngopharyngeal reflux with associated inflammation in the posterior larynx. This along with the yelling would have resulted in edema around the region along with pain. If you were to bring your mother to me I would suggest taking tablet Pan (Pantoprazole) 40 mg one tablet twice daily before meals for 15 days. Next, you should take syrup Gaviscon two tablespoon half an hour after each meal for 15 days. To relieve pain, she may take Dolo (Paracetamol and Hydrocodone) 650 mg one tablet once or twice daily as necessary. If she can pinpoint where she feels ulcer, she can apply Dologel CT gel over the area using clean fingers twice daily. If the ulcer is badly painful she can chew Lidocam (Benzoxonium chloride and Lidocaine) lozenges which relieve pain wonderfully. Initially, to remove inflammation I would also advise on taking Chymoral Forte (Diclofenac) tablet twice daily for five days. Consult a specialist doctor, discuss with them, and take medications with their consent. Looking at the pictures I do not feel there is an active infection that we should start her on an antibiotic. We shall observe her for three to four days and if the pain does not get relieved we can institute antibiotics for her. Secondly, she must follow some dietary restrictions as follows: 1. Avoid spicy and oily foods. 2. Take plenty of water but not immediately after meals. 3. Take adequate green leafy vegetables in diet. 4. Constipation must be avoided at all costs. You may try Ispaghula husk. It has to be mixed in two glasses of water and drank. 5. After that she must take few glasses of water before sleep. The husk adds bulk to stools and prevents constipation. If she has symptoms of foreign body feeling in throat or excessive mucus or nose block or post nasal drip we may have to add some more medicines. But for now, these medicines should suffice.
