Patient's Query

HelIo doctor,

I have attached the thyroid profile test reports of my mother who is taking Thyronorm 50 mcg currently. I want to know what is your opinion to control the thyroid functions.

Hello,

Welcome to icliniq.com.

I just read your query and from what you have mentioned it appears that she is under-corrected for hypothyroidism. You must increase the dose of Thyronorm (Thyroxine sodium) to 100 mcg once daily and repeat thyroid function test after four weeks and then we will review again and decide on whether there is any further need for dose titrations or not. Besides I would like to ask whether she takes her medications regularly early in the morning on empty stomach and then has breakfast one hour later? Secondly, is she on any other medication for any other condition? Also, does she have any gastrointestinal problem like problems with digestion, etc.? Also, please do a thyroid antibodies test as well, anti-TPO and anti-thyroglobulin antibodies.

Patient's Query

Hi doctor,

Thank you for your review and advise. I have attached the anti-TPO and anti-thyroglobulin antibodies test results. She takes her medications regularly early in the morning on empty stomach and then has breakfast one hour later for the last one and half year (before that she was taking 150 mcg few years, but it was just before her breakfast, when I came to know and suggested to have it in empty stomach and one hour before tea or coffee in the morning, post that I see her TSH came down less than 0.03 and we started reducing the dose to 100 mcg, 75 mcg, and 50 mcg). She is not taking any other medicine and no other problems, last time I did see HbA1c was 6.5 which need to be checked once again for prediabetes. There is no problem with digestion, etc. One more thing I want to highlight is she is not able to sleep in closed room or house even will not be able to take bath in bathroom which she feels incompatible, as we stay in farm house in the middle of our agriculture land we are able to manage and it becomes problem when she comes to city.

Hello,

Welcome back to icliniq.com.

Yes, I saw her report and her antibodies are according to her condition and nothing unusual about it (attachment removed to protect patient identity). I will still stick with my previous advice that you must increase the dose of Thyronorm (Thyroxine sodium) to 100 mcg once daily and repeat thyroid function test after four weeks and then we will review again and decide on whether she needs any further dose titrations or not.

Regarding her living patterns that is not a problem for her condition.

Same symptoms don't mean you have the same problem. Consult a doctor now!
