Hello doctor,

I am a 28-year-old male. I had sexual intercourse with my girlfriend a few days back, and my foreskin had come far down the penis head, which is painful. Also, my exposed penis is very sensitive to touch. This is because the foreskin, which has come beyond the penis head, is not returning. I also have a painful erection, and I do not feel comfortable like before. I am also getting some smell around my penis.

1. Can it be smegma?

2. What should I do?

3. Is this a serious issue?

4. Why is my foreskin not getting above my penis head and covering my penis like before? Is this normal?

5. Should I consult a doctor?

I am so much confused.

Phimosis means tight foreskin. In this condition, the foreskin may not be moving freely. There are two ways to treat it. One is medical, and the other is surgical. I will suggest medical treatment. First, pull down the foreskin as down as possible and apply Betamethasone cream on the inner side of the foreskin where it is tightly attached, do it three times daily for the next two to three months. Betamethasone will help to loosen the skin. You can consult your specialist doctor and use the medication with their consent. Also, do daily stretching exercises of the foreskin. Stretch the foreskin forward for 30 seconds, then stretch downward for 30 seconds, and do it daily for five minutes.
