Hello doctor,

My buttocks have some skin disorders. It is filled with a lot of small white pimples, and it gets dead, and the skin becomes very bad. It looks very dirty, and I feel very uncomfortable. No itching or pain, but rarely I feel itchy. In my teenage years, it started, I guess. Now, I am 28 years old. Please suggest the appropriate treatment. Please help.

Based on my assessment, it appears that you are dealing with tinea incognito combined with superadded folliculitis (a fungal infection of the skin that is not easily recognizable because it has been altered or masked by the use of topical treatments, particularly steroids). This condition requires specific care and attention to ensure effective treatment.

First, make sure to keep the affected area completely dry, as moisture can worsen the fungal infection. Use Clotrimazole powder frequently to maintain dryness, and change your undergarments regularly. Avoid excessive sweating and refrain from wearing wet clothes. Do not apply any general or steroid creams, such as Betnovate, as they may aggravate the condition. Even after the symptoms improve, continue the treatment for a longer period to eradicate the fungal infection completely. If the condition is recurrent despite proper treatment, it is essential to check for underlying issues like diabetes or thyroid problems, which could be contributing to the persistence of the infection.

For treatment, I recommend applying 2 percent Sertaconazole (Onabet) plus Fusidic acid (Fucidin) cream twice daily on the affected area. Additionally, take 500 mg of azithromycin once daily for five days. After five days, follow up for a review to determine the next steps in your treatment plan.
