My re-root canal treated tooth hurts when I touch it with my tongue. Why?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

The bottom last molar has been in a lot of pain. First, the crown fell off, and the top tooth moved down, so I had to have it adjusted. Then the tooth started hurting very badly, so I had to get a root canal treatment. After the root canal treatment, the constant pain is gone, but it hurts when touched (now with a permanent crown on) with my tongue. Also, every time I go to bed or when I wake up, the crown feels higher. I have been wondering if the tooth was cracked before the crown was put on.

Kindly advise.

Hello,

Welcome to iCliniq.com.

I understand your concern.

In cases of a tooth fracture, pain is usually felt only while biting. In contrast, if a dental restoration (filling) is over-contoured, meaning it is slightly higher than normal, pain is typically experienced during chewing.

If such symptoms are present, it is advisable to provide an update for further evaluation. If no discomfort is present, there is usually no cause for concern, as the restoration often settles and adapts over time.

The possible causes in this situation include an over-contoured restoration or a tooth fracture. An X-ray (radiographic examination) may be required if symptoms develop to confirm the diagnosis. Based on the current information, the complaint appears to be pain, which may be related to these causes.

If symptoms occur or persist, treatment may involve adjusting the restoration or managing a fracture if present. It is recommended to wait for five to seven days and monitor for any symptoms. If pain develops or continues, an update should be provided for further guidance.

I hope this helps you.

Thank you.
