I am a 22-year-old woman with dark circles, open pores, whiteheads, blackheads, and an uneven skin tone. My skin looks aged. What should I do?

I understand how upsetting it can be at 22 to feel that your skin looks uneven, aged, or tired because of dark circles, open pores, whiteheads, and blackheads. These concerns are very common, and the good news is that they are manageable with the right, gentle routine and consistency.
Cleanse your face twice daily with a mild, non-soap face wash, followed by a light, barrier-repairing moisturizer. Avoid scrubs, harsh exfoliants, or frequent facials, as these can worsen pores and make skin look older. Daily sunscreen is extremely important, even if you stay indoors, as sun exposure worsens pigmentation and enlarges pores over time. Apply it generously and reapply if exposed to daylight.
For your whiteheads, blackheads, and open pores, start with Azelaic acid 15% cream (Aziderm 15%), which is safe, effective, and well-tolerated. Apply a pea-sized amount to the affected areas at night, under a light moisturizer. Begin two to three nights per week and gradually increase to alternate nights or nightly use as your skin adjusts. Azelaic acid helps unclog pores, reduce comedones, improve uneven skin tone, and also gently lightens pigmentation without making skin sensitive or thin.
Regarding dark circles, they are often multifactorial. Genetics, thin under-eye skin, pigmentation, lack of sleep, stress, and nutritional deficiencies such as low iron or vitamin B12 can all contribute. It would be helpful to get serum iron and vitamin B12 levels checked. Make sure you are getting adequate sleep, staying well hydrated, reducing screen time, avoiding eye rubbing, and protecting the under-eye area from sun exposure.
For active treatment, apply an under-eye cream once daily at bedtime, using your index finger and very light tapping motions, without applying pressure. Use only a small amount, as the under-eye skin is delicate.
With regular care, patience, and consistency, your skin texture, tone, and overall freshness can improve significantly. Please give this routine some time and plan a follow-up to reassess progress and make adjustments if needed.
