Which treatment is best for my dad’s urethral cancer at 66?

Hi doctor,

My father, who is 66 years old now, underwent urethral stricture surgery 8 years ago. The treatment was done for his chronic problem of passing urine. Treatment by the doctor at that time ended up in adopting an alternate route for passing urine, which is below the testicles.

All was fine for six years, and he could pass urine properly only via a new exit route that is below the testicles. But two months ago, he discovered a wound (peri-urethral) near the place of the new exit. For this, the doctors again did a surgery and recommended a biopsy as well. The biopsy done has concluded in a well-differentiated SCC. The details of the biopsy report are attached here.

  1. What does the biopsy say?
  2. What kind of treatment option is available for his given urological history?

Your father has urethral carcinoma for which surgery is the best modality of treatment. He will need complete removal of the urethra and bladder, if necessary. It is a major surgery. If your father's health is fine, then he should be able to tolerate it.

Hi doctor,

Thanks a lot for your kind suggestion. My father's health seems to be fine. Last week, we took the consultation for further investigation. He suggested a CT scan of the pelvis and abdomen, blood tests, ECG, x-ray and verification of biopsy. He did hint at a major surgery, and we are awaiting reports. He also said that the treatment could be possible here.

In our next consultation, we are asked to visit again when he gets back all the reports from all departments.

  1. How will they know the stage of cancer, its gravity and extent?
  2. What is the survival chance in this case?
  3. What is the after-effect or impact of the surgery?
  4. Will it be recurring?
  5. What consultation would be required again?

CT abdomen and cystourethroscopy will help to stage the disease. Survival depends on the presence of lymph node metastases. As with any other malignancy, recurrence is possible. So, regular follow up is mandatory.

The tumor is confined to the urethra, and it has not metastasized anywhere. That is definitely good news, because surgery can cure his cancer now. The surgery will be a radical cystourethrectomy with an ileal conduit.

Thank you for your response. This is really helpful. Also, during the conversation with my doctor, he also mentioned that the surgery and post-surgical procedures could be time-consuming here (may span a few months). He did suggest that you could also take the opinion from private hospitals for faster treatment.

1. Are there any hospitals that you would recommend where we could get this surgery done as soon as possible?

2. Would you have any suggestions for reasonable private hospitals as well?
