Hello doctor,

I do have urinary urgency for ten years. The CT scan was done to check the abdomen for constipation. The result showed circumferential wall thickening in the urinary bladder, which can be seen with nondistention versus cystitis. Please explain.

I read your query and can understand your concern. Thickening of the bladder wall is generally a sign of bladder infection, UTI (urinary tract infection), or cystitis. Still, it can be a sign of chronic inflammation or bladder outlet obstruction. Please help me with a few details. Are you diabetic? Or do you have any other medical issues? Has your urgency constantly persisted for the last ten years, or does it come and go? How many times per day and night do you pass urine, and roughly how much is the quantity per void? Is the urgency severe enough to cause a leak on occasion before you can reach the bathroom? Do you have to strain to pee? Or do you have a feeling of incomplete bladder emptying? Do you have any pain in the loins or lower abdomen? Do you have any burning sensation, or have you had blood in your urine? Did you recently have a urine test? Please upload all possible reports for me to see, and I will do my best to help you.

Hi doctor,

Thanks for the reply.

There is no blood or pain when I urinate. I drink a lot of water, close to 3 liters. I did have a recent urine test done, but it was two months back. It was normal. I had an ultrasound of my bladder done one year back, but I am not sure why? Here are the results (I am worried about cancer like everybody else).

One year back, renal and bladder ultrasound result is mentioned below.

Indication: RLQ P. pain.

Comparison: None.

Technique: Transcutaneous imaging was performed.

Right Kidney:

1. Maximal length: 11.2 cm.

2. Cortical thickness: Within normal limits.

3. Echogenicity: Within normal limits.

4. Solid masses: None.

5. Cysts: None.

6. Nephrolithiasis: None.

7. Hydronephrosis: None.

8. Proximal ureter: No hydroureter.

Left Kidney:

1. Maximal length: 11.8 cm.

2. Cortical thickness: Within normal limits.

3. Echogenicity: Within normal limits.

4. Solid masses: None.

5. Cysts: None.

6. Nephrolithiasis: None.

7. Hydronephrosis: None.

8. Proximal ureter: No hydroureter.

Bladder:

1. Ureteral jets: Not visualized.

2. Filling defects: Evaluation is limited due to underdistention.

3. Wall thickening: None.

Impression: No significant abnormality.

I read your query and can understand your concern. The ultrasound report is normal, and as you mentioned earlier, only some thickening was seen on the CT (computed tomography) scan. If you have no blood in the urine or any other symptom other than urgency, the possibility of having cancer is next to none. I did forget to ask, are you postmenopausal? Do you have to strain to pass urine? Overactive bladder-causing urgency is common in postmenopausal women. Also, being constipated put pressure on the bladder. I suggest you take the medication for two weeks and revert to me about how you feel. Tablet Mirabegron 50 mg once before food, a stool softening syrup or tablet daily before going to bed, tablet Cranpac D (cranberry extract and D-mannose) twice daily or any other alternative available at your pharmacy, and tablet Econorm (Saccharomyces boulardii)250 mg once daily or any similar probiotic available at your pharmacy. However, please consult a physician, discuss with them and start taking the medications with their consent.
