Hi doctor,

I am a 27-year-old male, fitter than average but not quite a disciplined athlete. For many years, since my early teens, I have had to urinate frequently, more so than any peers. In recent years I feel it was gotten worse. Having to urinate more frequently, sometimes multiple times an hour, waking up through the night at least once, maybe two or three times, usually but not always, I sometimes sleep through. When I need to go, there is a great sense of urgency and a feeling of limited control slipping away; I can usually hold it back for a few minutes at best before I have to run to relieve myself. It is not always easy to calculate, and sometimes I am not quick enough either cut it so close that the first few drops or spurt escapes, but often sufficient, a greater stream will escape before I made it to the toilet. I will usually find somewhere more appropriate than having such accidents but frequently end up reaching for a jar or disposable cup, even plastic bags at times. It interferes with the flow of life every day.

Even before, during, and right after sexual intercourse with my partner, I have to urinate. And as far as sexual performance, I also suffer from premature ejaculation. The severity differs, but occasionally, I feel I live up to my body's capabilities, rarely lasting more than a minute or so, sometimes having more control, but having to exert so much focused energy into the body to hold on. The last couple of times, it was a little better, but I wonder if there is some connection between the urinary incontinence and the difficulty holding back ejaculation. I am sure there is more helpful information in me that may or may not interplay, but those are the key points for now. Any advice is much appreciated. I currently take Vitamin D, Magnesium, and Zinc supplements. I use to eat low sugar and low carb diet.

You are a 27-year-old male with urgency and urge incontinence in passing urine a few times in one hour in the day and at least once at night. Premature ejaculation is another issue. The first issue, since you are a careful young work upstart from bladder diary to uroflow bladder scan, flexible cystoscopy, and bladder biopsy if required. Investigations like urine analysis, urine culture, and ultrasound are all required. Causes can be like urinary infection, urinary stones, or cystitis, which is the bladder's inflammation. Treatment depends on the cause. A bladder diary is to record every time you pee how much comes out for 24 hours and two consecutive days. This can show many things, like bladder capacity and frequency. In addition to that, uroflowmetry can show the pressure of urine flow and the pattern of it. Flexible cystoscopy is done as an office procedure. Still, it is of great value and shows all lower urinary tract anatomy to take a biopsy for an accurate diagnosis. Please get the workup mentioned above done and follow up so I can decide on treatment. For premature ejaculation, it can be linked to the urgency in some causes like infection or stones, especially if this premature ejaculation started recently will update you on how to treat both issues after having worked up done.
