Patient's Query
Hi doctor,
I experienced sharp pain in my urethra eight months ago while urinating, accompanied by blood for four days. A pint of blood flowed from the urethra after urinating. I underwent an ultrasound and a microscopic urine test, but no blood was detected, ruling out UTI, Gonorrhea, and Chlamydia. The ultrasound results were normal.
Three months later, a cystoscopy revealed nothing significant, although the doctor noted some minor tissue scars. By the fourth month, I started experiencing regular hives, which lasted for more than six weeks. The doctor recommended antihistamines, which I have been taking for over six months, but no infections have been diagnosed. I also tested for syphilis, hepatitis C, and HIV, but the hives persist.
Kindly help.

I understand your concern.
I looked over the image (attachment removed to protect the patient's identity) you have attached. I would like to know a few additional details for a better evaluation.
Have you noticed any weight loss or loss of appetite, and have you experienced any low-grade fever?
Could you please attach a photo of your hives?

Patient's Query
Hi doctor,
Thank you for the reply.
There was no weight loss or loss of appetite, but there was a moderate fever at the beginning when there was blood in the urine. Small pimple-like boils are seen all over the body, and a black patch appears after three to four days.
Kindly suggest.
Hello,
Welcome back to icliniq.com.
I understand your concern.
I have gone through the images you have attached (the attachments were removed to protect the patient's identity). It is urticaria, a skin rash caused by an allergic response to food, drugs, or other irritants.
I would want to know if they would improve immediately after taking an antihistamine drug. May I know which antihistamine medicine you are taking?
Have you done blood tests such as absolute eosinophil count and immunoglobulin E (IgE) level?
Kindly revert with the answers.
I hope this helps.
Thank you.
Patient's Query
Hi doctor,
Thank you for the reply.
I am taking Cetirizine. The hives disappear for two days after using antihistamines, then reappear until I take the medication again. I have not done any other blood tests.
Please help.
Hello,
Welcome back to icliniq.com.
I understand your concern.
It would be best if you started taking tablet Montelukast 10 mg and tablet Fexofenadine instead of plain Cetirizine. These are anti-allergy medications. It is particularly effective in treating urticaria.
Kindly follow up with the answers.
I hope you find this helpful.
Thank you.
Patient's Query
Hi doctor,
Thank you for the reply.
I consulted a doctor for allergies and had a skin prick test. The test results are attached. He advised me to take Reactine for three months, but it is still ineffective since I have hives and itchy skin.
When should I begin using Allegra medicine, and for how long should I take it?
I have not taken these blood tests. Please suggest how I take it.
Thank you.
Hello,
Welcome back to icliniq.com.
I understand your concern.
I have reviewed the attached report (attachment removed to protect the patient's identity). It is better to avoid dust wherever possible. An absolute eosinophil count and an immunoglobulin E (IgE) test are required and can be done anytime. Fasting is not required for this.
Will you be able to get Allegra M without a prescription?
Kindly revert with the answers.
I hope this helps.
Thank you.

Patient's Query
Hello doctor.
Is there anything concerning in the reports? Since my hives have not stopped for more than seven months and due to medicines, hives are not showing up. But after a day of not using the medication, hives will start again. I also have pain in the chest (right side) and right-hand pain, and the doctor here told me to use an ulnar nerve brace at night.
Thank you.

Yes, the reports are good. So be relaxed and calm.
