Hi doctor,

I had a sharp pain in my urethra eight months ago while urinating, and there was blood for four days. A pint of blood flowed from the urethra after urinating. I underwent an ultrasound and microscopic urine test, and no blood was detected. Therefore, it is neither UTI, Gonorrhea, nor Chlamydia. The ultrasound result revealed nothing. After three months, I had a Cystoscopy, and the doctor discovered nothing. However, he mentioned some minor tissue scars, and later in the fourth month, I started getting hives regularly, which lasted for more than six weeks. The doctor suggested nothing other than an antihistamine. Thus I have been taking an antihistamine for more than six months, but no infections have been diagnosed. I had done syphilis, hepatitis C, and HIV tests, but my hives continued to exist.

There was no weight loss or loss of appetite, but there was a moderate fever at the beginning when there was blood in the urine. Small pimple-like boils are seen all over the body, and a black patch appears after 3 to 4 days.

I understand your concern. I have gone through the images you have attached. It is urticaria, a skin rash caused by an allergic response to food, drugs, or other irritants. I would want to know if they would improve immediately after taking an antihistamine drug? May I know which antihistamine medicine you are taking? Have you done blood tests such as absolute eosinophil count and Immunoglobulin E (IgE) level?

I am taking Reactine. The hives disappear for two days after using antihistamines, then reappear until I take the medication again. I have not done any other blood tests.

It would be best if you started taking tablet Montelukast 10 mg and tablet Fexofenadine (Allegra) instead of plain Cetrizine. These are anti-allergy medications. It is particularly effective in treating urticaria. Have you consulted a dermatologist regarding this issue? Can you get an absolute eosinophil count, Immunoglobulin E (IgE), and allergy testing done?

I consulted a doctor for allergies and had a skin prick test. The test results are attached along. He advised me to take Reactine for three months, but it is still ineffective since I have hives and itchy skin. When should I begin using Allegra medicine, and for how long should I take it? I have not taken these blood tests. Please let me know how may I take it.

An absolute eosinophil count and Immunoglobulin E (IgE) test are required and can be done anytime. Fasting is not required. Will you be able to get Allegra M without a prescription?

Is there anything concerning in the reports? Since my hives have not stopped for more than seven months and due to medicines, hives are not showing up.
