Patient's Query
Hello doctor,
I am 34 years old and have been experiencing daily episodes of large, painful hives appearing unpredictably across different areas of my body for nearly two years, despite being on the maximum recommended dose of antihistamines that my dermatologist prescribed without conducting any investigation into what might actually be triggering or perpetuating these completely debilitating and treatment-resistant daily episodes.
Can a general physician help investigate whether persistently uncontrolled chronic urticaria may be driven by an underlying autoimmune condition, thyroid antibody positivity, or chronic infection?
Could these potential causes have remained undiagnosed because they were never properly screened for or systematically ruled out during two full years of ineffective antihistamine-only therapy?
Kindly advise.
Thank you.
Hello,
Welcome to icliniq.com.
I am really sorry this has been going on for so long, because daily painful hives that do not respond to treatment can be both physically exhausting and emotionally draining. The condition being described fits difficult-to-control chronic urticaria, and when symptoms persist despite high-dose antihistamines, it is completely reasonable to look deeper rather than continuing the same approach without investigation.
A general physician can absolutely help initiate a more thorough evaluation to look for contributing factors, such as autoimmune activity, including associations with thyroid conditions like Hashimoto's thyroiditis, as well as screening for infections or other systemic issues that may be perpetuating the hives.
Normally, specific blood tests are conducted rather than general tests without any focus, and in most cases, there is no clear cause identified. It is nonetheless crucial to rule out the possibilities of such conditions.
It is also worth knowing that when antihistamines alone are not enough, there are well-established next-line treatments that go beyond them and can significantly improve symptoms, so there are still meaningful options even after two frustrating years.
I hope this is clear, and if you have any questions at any time, please feel free to ask
Thank you.
