What could be the reason for unexplained weight loss, irregular menstrual cycles, and painful sex?

Hello doctor,

I recently had a hip arthroscopy due to a labral tear and had a repair and chondroplasty. I have been experiencing deep right pelvic pain, low back pain, and leg pain of a non-dermatomal pattern for over a year. Since surgery, the hip has improved, but I still have other pain. I have also lost 16 lbs since last year. This was unexplained and put down to stress as I had a lot going on at home and work. Over the year, I have experienced irregular menstrual cycles ranging from 11 to 45 days. I tested my ovulation for two months (urine test stick) and did not ovulate despite having a period. I find sex incredibly painful, and as I am now in my early 30s, I am slightly concerned about starting a family. Is it possible that I have a cyst or fibroid causing these symptoms?

You need to have an overall workup for your generalized and localized body health. Have a complete check of all medical issues and take regular and complete treatment accordingly. Your surgery actually included nerves as well, and those damaged nerves during surgery still have pain. Healing might disturb menstrual cycles due to weight loss, weight gain, strenuous exercise, prolonged illness, and medications.

1. Take a healthy diet with lots of fruits and vegetables.

2. Use less salt, sugar, oil, and spices.

3. Take a daily walk, exercise, and maintain a healthy weight.

4. Try to have physiotherapy for pain management, as nerve pain cannot be completely relieved by even analgesics.

5. If you want regular cycles, take combined oral contraceptive pills for three months after consulting your gynecologist.

6. Have a transvaginal scan to exclude any pathology.
