I had sex after a long time the day before yesterday. When I go to the washroom to urinate, it is burning and somewhat paining. Please suggest what to do and what the reason behind it could be.

The symptoms sound like some vaginal infection. Vaginal infection is transmitted during sex (so the partner is the source), so both have to be treated at a time, or else you will get infected the next time of sex. You may have white or yellow, or grayish discharge with or without itching, foul smell, pain at the beginning of sex due to dryness (as the lubrication will be reduced) late towards the end when a fully erect penis touches the innermost vagina due to inflammation of the structures around the uterus with pain radiating to the back and thighs. Associated with infection, the bowel movement is also affected. There are also features of gastritis.

Bleeding of periods will be thicker and darker, with clots and fleshy masses like, and may smell bad; there are more days with spotting after the periods, too. When you have an infection inside the uterus, the blood flow increases as the inner lining is inflamed, and the blood mixed with the inner lining of the uterus forms a thick, doughy, or fleshy mass. As the healing of the lining is not complete, it keeps spotting, or you have blood-stained discharge. Pain is throbbing even before and lasts after periods are over, too. Usually, the infection is mixed and not just due to yeast fungus. There will be a family of aerobic and anaerobic bacteria, which cannot be detected on culture. When one is treated and cleared, the other will grow, and if not fully treated, it will regrow. Before starting the treatment, confirm that you are not pregnant. I suggest you and your partner consult your specialist doctor, discuss with them, and, with their consent, take the medicines mentioned below.

1. The Fluconazole tablet is 150 mg, and there is only one tablet with no repetition after lunch.

2. Tablet Azithromycin 1 gm stat only one tablet with no repetition one hour before food.

3. Tab Secnidazole Forte, two tablets stat with no repetition after dinner.

All these will be done on the same day by both of you. I suggest you can also have capsule Doxycycline 100 mg twice a day for one week, vaginal wash with V Wash (Lactic Acid Formulation), or similar cleanser twice a day and a vaginal pessary. Also, capsule Clingen Forte (Clindamycin, Clotrimazole, and Tinidazole) or Candid-CL (Clindamycin Topical and Clotrimazole Topical) or Cansoft-CL (Clindamycin and Clotrimazole) once at night for five nights. Your partner can have a local application of an antibiotic and antifungal cream Candid-B (Beclometasone Topical and Clotrimazole Topical) or Candiderma cream (Beclometasone Topical, Clotrimazole Topical, Neomycin Topical) or Lobate GM (Clobetasol Topical, Miconazole Topical, and Neomycin Topical) on the penis twice a day after wash for seven days.

Do not have sex until the infection is fully gone.

I used proper protection, and we cleaned up before and after the intercourse. My partner did the same. We still fail to understand what went wrong. My periods had ended just the day we had sex. Could this have had any impact?

There is no relationship to the day of periods. No matter how clean you and your partner are, there will be some mix-up of secretions of genital fluids, which can cause infections. Other possibilities I can think of are:

1. If you were having sex after a long time, there can be a few minute tears in the vaginal wall, which can irritate you.

2. If you are allergic to latex th, the material of the condom th,e spermicidal gel, or any new type of condom.

3. If you feel like this infection is not severe, just have a vaginal wash with V Wash (lactic acid formulation) or IntiWash (lactic acid, sea buckthorn oil, and tea tree oil) or HiWash or Gynact (Leuprolide/Leuprorelin) twice a day for seven days. Consult your specialist doctor, discuss with him or her, and with their consent, take the medicines.

Do wash after the bath and before going to bed both the outer and, as far as possible, the vagina gently. You can also use vaginal suppositories. You will be relieved within two to three days.

I have the first three medicines: Fluconazole 150 mg, Secnidazole 1 g, and Azicip 500 mg. Kindly suggest their intake once again. Also, the pain while urinating has increased.

I suggest you take the tablet Azicip (Azithromycin) before food, Fluconazole after food, and Secnidazole at bedtime. You need lots of water and fluids like juice mi,lk ba,rley water, or curd and keep passing urine frequently. So, the infection will flow out. At bedtime, use suppositories too. You can also use the vaginal washes that I have mentioned. Suppose you feel pain in the lower abdominal area. In that case, you can use a painkiller like Meftal Spas (Dicyclomine and Mefenamic Acid) or Cyclopam (Dicyclomine and Paracetamol) and Mensospa (Ashwagandha, Natural Calcium, Soy isoflavones and Shukti bhasma). Consult your specialist doctor, discuss with him or her, and with their consent, take the medicines.
