Hello doctor,

I am a 21-year-old female, 5'7" and 170 lbs. I am experiencing itching and discomfort in my vaginal area, along with a white discharge. I suspect I may have a yeast infection.

I understand your concern.

Sorry for what you are having. Itching in the vaginal area can be very discomforting and also embarrassing, especially in public. The white discharge you are having is, as you said, a yeast infection (vagina candidiasis).

It usually starts from the top of the vagina, from where the discharge spreads down to the vaginal opening and vulva, causing the itching you are having.

So, the treatment would involve using a vaginal antifungal pessary or suppository and an antifungal vaginal cream.

The pessaries would be inserted into the vagina and pushed to the top, so that it treats the actual area where the infection starts. The vaginal antifungal cream would be applied twice a day at the entrance of the vagina and the vulva, which would help reduce the itching.

Also, avoid sexual intercourse during the period of treatment, and the vaginal inserts are better inserted at night so as not to interfere with your daily activities.

The vaginal insert is better, however. If you are not comfortable with vaginal inserts or are still a virgin, let me know so that I can prescribe oral alternatives to the vaginal inserts.

The probable cause is vaginal candidiasis, and the investigations to be done are STI (sexually transmitted infections) screens. The treatment plan includes Clotrimazole, Clindamycin, and Metronidazole vaginal suppositories for six days and Clotrimazole vaginal cream twice a day for 10 days, and avoid sexual intercourse during the period of treatment.
