Hi doctor,

Today I am too much worried when I see my blood report. My vitamin B12 level is too low, 215 pg/ml, as the reference range was (211 to 911). I feel headaches on both sides, pressure in my head, and exhaustion. I want to sleep all the time. The other thing is I have high hemoglobin 17 g/dl and high HCT 51.1%.

  1. Since I eat more eggs, fish, and meat, how do I get deficient in B12?
  2. Do all my symptoms belong to vitamin B12?
  3. Do you think I have polycythemia because of high hemoglobin, HCT, and LOW B12?
  4. Are all these indicators of polycythemia?
  5. How vitamin B12, HCT, and hemoglobin are related to polycythemia?
  6. Do you think I have to go to a hematologist for further evaluation and treatment for polycythemia?

In normal adults, normal male hemoglobin value is between 13 to 17 gm/dl. The hematocrit test (Hct) value is between 40 to 50 %. So your HB value is normal, which is only slightly raised, which might be secondary to dehydration.

The chance of polycythemia (primary polycythemia) seems less, per the report (attachments removed for patient identity protection). Repeat the complete blood count after a week. If hemoglobin value increases and Hct rises, the Jak2 mutation study can be done as a workup for primary polycythemia. But just now, I do not think you have primary polycythemia.

Your B12 value is within range (because the reference range provided is 211 to 911 pg/mL). The value of any report should be evaluated according to the reference range provided in the report because different laboratories have different instruments and work on different principles.

Your headache should be evaluated for possible causes according to examination.

Check blood pressure as it can lead to vascular headaches. Check for ear pain history and ear examination to rule out an otitis-like ear infection. Tooth infection, sinus inflammation, and viral upper respiratory tract infection can also lead to headaches. Your neurological examination should be done. If needed, then a computed tomography scan can be done. Hypoglycemia, lack of sleep, excess stress, alcohol, and smoking can lead to headaches. According to the cause of the headache, specific treatment is guided. Ibuprofen or Rizatriptan might be prescribed if you have a migraine history.

Avoid stress if taking it because it can lead to tension-type of headaches. So evaluate your headache by physical examination by a nearby physician.
