Hello doctor,

This is about my white spot issue. I have one white spot on my leg due to an accident, which is not getting its color back. So I did a tattoo on it. The tattoo maker also did not use perfect color and it was light in color. I started Excimer treatment on it but I see very little color came back on 2nd treatment, a total of 6 treatments are done. My questions are,

  1. Does excimer work even if there is a tattoo on the skin?
  2. If it is working, is it possible that we cannot see the color because it is behind the tattoo?
  3. There are two spots in my hands too, it looks like the excimer not working there either. I thought excimer works fast and I will see color soon. How much time does excimer take to work?

I understand your concern.

Excimer laser in vitiligo works fine, but it takes 2 to 3 months to work properly. It should work irrespective of the tattoo over it as it is a type of focused phototherapy, which induces pigment formation. I cannot see much of the tattoo in the photograph so it is unlikely that it is hiding any pigmentation, besides the tattoo pigment is in the dermis, which is the deeper part of the skin and the pigment induced by vitiligo treatments is in the epidermis, which is the upper part of the skin.

If you have not tried other treatments, you can apply Tacrolimus 0.1% ointment at night and Flucort-H cream once in the morning, for six weeks and then follow up.

Hello doctor, thank you for the reply.

I do not have vitiligo. The scar on my leg is due to an accident. A few small spots on my leg and hand I have are because of an allergic condition. When I eat allergic things I get redness and itching. And at those places sometimes I get small white spots. I am getting phototherapy for that. Do both creams work in both types of conditions (allergic spots and scars)?

In the image, the whole pink area is tattooed, to hide the white spot tattoo maker person tried to use color but they could not make it match with skin and the color was light pink. After I get the color back I am thinking of removing the tattoo with laser treatment.

It is quite depigmented for me. Vitiligo at the site of trauma is quite common. Anyways if a dermatologist in person said that it is not Vitiligo, Tacrolimus 0.1% ointment along with Melgain lotion can be tried. That should help in repigmentation unless there is scarring. Phototherapy can be continued with these creams.
