Hi doctor,
I had vitrectomy for floaters six months ago. Because of that, I now have distorted and wavy vision. I have a long history of eye problems. I was born with congenital cataracts. This is all in my right eye. The left eye is legally blind. I have attached my optical coherence tomography (OCT) and eye pictures. Kindly help.

I have seen your optical coherence tomography (OCT) image (attachment removed to protect the patient's identity), which is suggestive of discontinuation of the main layer which is responsible for vision. There is epiretinal membrane seen overlying your retina which sometimes cause distortion. In your case, it is very fine, and does not need any treatment for it. Since you have one eye functional and post vitrectomised, I would suggest keeping a close watch on this symptom, if it increases consult a doctor physically and repeat OCT again after two months. I would suggest you follow the following treatment plan -Home Amsler chart. Eye drop Acular LS (ketorolac tromethamine ophthalmic solution) three times a day for two months. Eye drop Systane ultra [Polyethylene Glycol (0.4 % w/v) and Propylene Glycol (0.3 % w/v)] three times a day. Consult your specialist doctor, discuss with them and take medicines with their consent. I have seen your further optical coherence tomography (OCT) reports (attachment removed to protect patient's identity), and based on that, I diagnose it as a myopic refractive error. I would suggest you have OCT angiography. I want to see the macula area and the area between the optic nerve and macula. Thank you.

Thank you for the response. What do RBIs mean? I do not have a natural lens in the eye. Ever since I had the vitrectomy I see everything wavy. The drops you recommended are for what purpose? Will that help with the waviness? Kindly suggest.

I have seen (attachment removed to protect patient's identity) your optical coherence tomography (OCT) angiography results, which are fine; this must be old. They are fine. Vitrectomy was done only for floaters. In the high-definition OCT, you have shared, I am not happy with the continuity of the photoreceptor layer, which is responsible for waviness. These drops sometimes help decrease distortion and provide a soothing effect to the eye. Please continue to use these drops for one month. The contour of your eye is myopic and seems you would have in past high numbers. If you see your OCT, there is a bump seen, for which I wanted to see angiography, which is fine. Thank you.

I am attaching pictures of my retina before the surgery. This surgery really messed me up. Every doctor tells me something different. My doctor here tells me it was choroidal folds. Another one told me foveal atrophy. Another told me about retinal wrinkles. Another one told me about staphyloma. The big question is will the wavy vision go away and what can I do to treat it? This all happened after surgery. My doctor told me it will go away but it has been six months now.

Optical coherence tomography (OCT) reports (attachment removed to protect the patient's identity) show continuous retina layers. No disintegration was seen pre-surgery. I would suggest putting those drops as suggested. I do not feel any further intervention is needed, as the outcome will not change. Thank you.

The doctor I met today told me I have fluid under the retina. He recommended the eye drops but also told me Avastin [240 mg α,α-trehalose dihydrate, 23.2 mg sodium phosphate (monobasic, monohydrate)] injections would help. Any suggestions?

I have seen your optical coherence tomography (OCT) reports (attachment removed to protect patient's identity), and I could not see any fluid or choroidal no vascular membrane. Even in your angiography, there is no leakage seen. That is why I have suggested OCT angiography which is a three-dimensional view of the retina, What you have sent me is standard angiography. Moreover, your distortion is quite an old complaint, by that time the changes would have been seen in OCT. I still suggest you same as what I have advised. Thank you.
