Hi doctor,

I have been experiencing reddening at the tip of my penis, and there are white lesions inside my urethra. It is a little sticky, and at the end of urinating, it dribbles out. There is no pain or discomfort. It has been three weeks now. STD tests came back negative. I was prescribed Azithromycin and Doxycycline first, did nothing, and now I have been prescribed an anti-fungal cream (Clotrimazole) which has been five days and still no change. I am attaching an image. Kindly help me.

I can see a whitish slough inside (attachments removed to protect the patient's identity). As your urine flows well and no history of stones and STDs (sexually transmitted disease) were negative. The only thing left is a swab for culture and urine for culture. I would appreciate it if you do an ultrasound KUB (kidney, ureter, and bladder). I do not see any other cause for it. If you want to do a swab for culture and urine culture, then you need to be off antibiotics for 48 hours. Please follow up with tests.

Yes, stones can cause slough without symptoms. Those are whitish material which forms due to bacteria around the stones and comes out in urine through the urethra. Ultrasound KUB (kidney, ureter, and bladder) can help to detect if there are still stones or recurring stones are even there. About 50 percent of treated stones come back within ten years. We will need urine culture as well to check whether the infection is there.

The cream you use is for topical yeast infection. Your whitish slough comes from inside the urethra. Urine culture and swab will inform us whether it is bacterial infection or yeast. But anyway, I do not think the cream will be helpful. The problem is in the urinary tract, not the skin outside. I will always be there if you need any further assistance.
