Hello doctor,

My 5-year-old boy had a white secretion from his penis, and he experienced swelling and pain while urinating. Three days ago in the evening, my son developed these symptoms after urinating, along with a white secretion. His penis is swollen and sore. Currently I am applying antibiotic cream. Kindly help.

  1. Is your son circumcised? - No, he is not circumcised.
  2. If not circumcised, does his foreskin move back and forth easily? - No, the foreskin is very sore and does not move back and forth easily.
  3. If he is not circumcised, does his foreskin balloon up when he is peeing? - No, it has not ballooned up when peeing.
  4. Has he ever had this issue before? - No, he has not had this issue before.
  5. Is there any history of insect bite recently or him playing outside in grass or a plantation area? - No, there is no history of recent insect bites or him playing outside in grass or a plantation area.
  6. Does he have a fever? - No, he does not have a fever.

I read your query and understood your concern.

I have reviewed the photos you attached (attachments removed to protect the patient’s identity), I can see from the photo that it is quite inflamed. My guess is that he has had an insect bite which has caused this. Although he may have phimosis as well, if his skin was not retractable before this happened. I suggest you consult a specialist, talk with them, and take the medications with their consent. I suggest you follow the below-mentioned instructions for the time being to reduce the inflammation:

  1. Paracetamol syrup 5 ml three times a day for five days.
  2. Using a cold flannel to soothe the area.
  3. Use a steroid-based ointment. Take just a pea-sized application and apply it to the affected area twice a day for tow to three days.

Once this is settled, Kindly check if the foreskin retracts back, if not I suggest he undergo a circumcision. The white discharge is just the smegma which has accumulated underneath. If the skin starts to retract once the inflammation has settled, you can clean it off. I hope this has helped you. Kindly follow up if you have more doubts.
