Hello doctor,

I am worried whether my wound is infected. Three days back, I got a wound due to a sudden cut by a knife on my right hand. The wound is between the right thumb and my palm. It was really painful, and I cleaned the wound and applied Mupirocin cream. Should I apply a band-aid over it, or should it be left open? I feel it is infected, as I can see a yellow color fluid over my wound. It is not painful now; however, I am worried about the possibility of an infection. How to identify that the infection is not caused by Vibrio vulnificus? Please help.

I have gone through the details that you have provided. Following an injury, depending on the depth of the wound and based on the medications used, it is common to have infections. Do not worry since the possibility of having a Vibrio vulnificus infection is only a considerably rare possibility.

From the details provided, they appear to be a regular bacterial infection. Infections with Vibrio vulnificus cause other symptoms like fever, chills, headache, abdominal pain, cramps, and so on. The concern you are facing does not appear to be so. Also, kindly provide a clear image of the affected area as it would be helpful to assess the wound better. I suggest you do the following:

  1. Take a capsule of Amoxicillin and clavulanic acid (Augmentin) 625 mg after dinner for five days. This will help in faster recovery. Make sure there is no previous history of allergy to Amoxicillin before consuming this capsule. In case of allergy, do not consume, and may be an alternative can be considered.
  2. Consume one tablet of Pantoprazole 40 mg 30 minutes before breakfast and dinner for five days.
  3. Apply Fusidic acid (Fucidin) cream thrice daily on the affected area.
  4. If the wound is deep, thorough cleaning with sterile saline water and Betadine solution every day along with dressing is advised for at least five days. I advise dressing to be done only in a hospital setting.
  5. I would highly advise a direct consultation with a GP or a specialist for examination in person in order to check for other signs of infection and testing of swabs for understanding the exact bacteria that may be present.
  6. Within 72 hours of injury, a TT (tetanus toxoid) injection is advised. Kindly note this for future reference as well. I highly recommend a direct consultation with a trauma specialist.
