Should a Rh-negative mother terminate her second pregnancy?

Hi, Welcome to icliniq.com. Some more points regarding her situation and previous history are needed actually. She is Rh negative and husband Rh-positive but is it known that he is homozygous or heterozygous for the anti-D allele? If homozygous, then the baby would be Rh-negative and have more chances of complications, but if heterozygous, more chorionic villus sampling or amniocentesis is needed to know the child's blood group. Was the previous child's blood group known? Was it positive or negative? An antibody titer of 1:4 is considered alloimmunized and needs a monthly antibody titer, whereas an antibody titer of 1:16 needs strict monitoring. The baby has more chances of having fetal anemia, hydrops fetalis, jaundice, etc.

What could cause no yolk sac in the gestational sac?

Hi, Welcome to icliniq.com. You are receiving enough supportive treatment. In case of discrepancy between dates and scan, yolk sac and fetal pole could appear but there is continuous brown discharge which is risk actually. Have a complete avoidance of restricted activities that can trigger a miscarriage, like stairs, stress, sex, abnormal blood pressure, sugar levels, thyroid levels, hormonal imbalance, etc. Have a strict check on symptoms like the passage of clots, bleeding with or without pain, etc.

Please look at TORCH test results and tell if there is any concern.

Hello, Welcome to icliniq.com. I have gone through your wife's TORCH (toxoplasmosis, rubella, cytomegalovirus, herpes simplex, and HIV) test reports. There are two types of antibodies that were investigated, IgG and IgM, to exclude infection. The result could be positive or negative, depending on the findings of IgG and IgM. A positive test result means IgG and IgM antibodies are found in one or more infections covered in screening.

Can I miss my periods even if I am sexually active?

Hello, Welcome to icliniq.com. I read your query and can understand your concern. You have cramps with missed periods after unprotected sex and are on multiple medications, so it is better to know your pregnancy status correctly. Have serum beta HCG (human chorionic gonadotropin) immediately. If it comes positive, it is better to repeat it after exactly 48 hours to check the progression of beta HCG (human chorionic gonadotropin).

How to manage PCOD and thyroid-related weight gain at 24?

Hi, Welcome to icliniq.com. Thyroid and PCOD both affect fertility so need precautions, investigations on regular basis if needed, treatment and dose adjustments accordingly. Thyroid abnormalities can affect fertility, periods flow regularity, can cause miscarriages, increases risks of congenital anomalies in fetus, etc. So have regular consultations and check of thyroid levels so can have dose adjustments. Polycystic ovaries can disturb periods scanty / irregular, can give abnormal hair growth, sudden weight gain, disturb insulin levels that increases risks of having diabetes as well as disturb metabolism.

Can a small thin-walled hemorrhagic cyst turn cancerous?

Hi, Welcome to icliniq.com. I have seen your attached report (attachment removed to protect patient identity). It would be better for me to have your mother's obstetrics, gynecological, medical, surgical, and drug present and past history as I could be able to correlate symptoms. If she has been treated for any problem, it is better to have a check on that and take regular treatment like diabetes, hypertension, etc. This perimenopausal age is very much prone to get disease due to aging.

4 ultrasound after IVF shows no fetal pole. Please help.

Hi, Welcome to icliniq.com. It seems it was anembryonic pregnancy means the sac was formed but without a fetal pole. Sac is not growing inside to have a fetal pole and gradually towards further stages of pregnancy even after hormonal support. It is a bit depressing but if you want to wait for some time more to have nature to react rather then your own initiative then you can quit hormonal support and wait. Cramps indicate that natural miscarriage is soon to happen.

I'm 40. What tests are needed before pregnancy?

Hi, Welcome to icliniq.com. There is risk involved for conceiving at this age. Age-related screening is needed before conception even if you both are healthy and do not have any problem. As long waited oocytes (eggs) will be involved in conception, there are a few tests for conception at this age to check baby either it is free from anomaly or not like chronic villus sampling, amniocentesis and double, triple or quadruple tests to check early conception for excluding fetal anomalies. I am not confirming any mishap but it is the only recommendation for lots of precautions.
