How to manage PCOS/PMOS and its related symptoms?

Hello, Welcome to icliniq.com. I read your query and can understand your concern. Your friend is suffering from polycystic ovarian syndrome/polyendocrine metabolic ovarian syndrome (PCOS/PMOS). In this disease, the periods become irregular, and there is difficulty in conceiving a baby, which may be the cause of her suicidal thoughts. The hormonal imbalance can be treated by regular hormone therapy for at least three months.

What are the different tests available to detect PCOS/PMOS?

Hi, Welcome to icliniq.com. The cause of delayed periods may be due to stress, anemia, or thyroid dysfunction. I suggest getting a complete blood count and a thyroid profile done and reverting with the reports. In the case of overweight, I also suggest checking the levels of LH (luteinizing hormone) and FSH (follicle-stimulating hormone)..

How can a woman with PCOS/PMOS conceive safely?

Hi, Welcome to icliniq.com. PCOS (polycystic ovary syndrome)/PMOS (polyendocrine metabolic ovarian syndrome) is a condition in which multiple cysts form in the ovary along with acne and weight gain. It is a very good sign that you have reduced your weight to 126 pounds, which in itself removes many of the complications of PCOS/PMOS, like diabetes mellitus. Hormonal changes, which are a part of PCOS/PMOS, may cause some difficulty in conceiving. But there are treatments available to help in this process.

What could be the reason for persistent clumsy discharge and vaginal dryness despite treatment for pelvic inflammatory disease?

Hi, Welcome to icliniq.com. As you have already taken the whole treatment of pelvic inflammatory disease (PID) along with other tests like HIV (human immunodeficiency virus) I do not think you require any other treatment for the dryness around the vagina and not inside the vagina. It is basically due to the course which you have taken leading to a decreased immunity. I would suggest you follow medicines which will help you to boost your immunity. You have the clumsy discharge which should also go away and the only thing is that there should not be any foul smell in the discharge.

What does an hCG level of less than 0.1 mlU/mL indicate?

Hi doctor, Welcome to icliniq.com. I have seen the attachment (attachment removed to protect the patient's identity) and it shows your hCG (human chorionic gonadotropin) level is 0.1 million international units per ml, which is within the normal range and so there is no chance of pregnancy. As you have done the test for thyroid as well as diabetes, I would suggest by seeing the clinical report you are overweight rather than obese. I would suggest you get an ultrasound of your lower abdomen done to rule out polycystic ovarian disease which also causes delayed periods apart from obesity and acne over the face.

Can Levothyroxine cause heavy and painful bleeding?

Hi, Welcome to icliniq.com. First of all, I would like to advise you to never stop any medicine on your own because these medications can have sudden withdrawal symptoms, which can be harmful. Always consult your treating doctor before starting or stopping any medicines, because these medicines have side effects on use. I understand your concern. As you have stopped taking the tablet Levothyroxine 75 mg, and again started taking it after a period of amenorrhea, the bleeding started after restarting the tablet Levothyroxine 75 mg, is because of hormonal imbalance and changes in the thyroid levels T3 (triiodothyronine) and T4 (thyroxine).

What is causing my severe menstrual cramps and back pain?

Hi, Welcome to icliniq.com. The pain during the periods is basically due to dysmenorrhea. Such conditions occur when we eat more spicy foods and stress too much. I suggest you get an ultrasound of your lower abdomen done to rule out any organic pathology, like an ovarian cyst or pelvic inflammation. Otherwise, you should eat more vegetables and fruits.

What are the treatment options for RPOC: D&C or wait?

Hello, Welcome to icliniq.com. I have gone through the reports attached. (attachments removed to protect patient's identity) According to the ultrasound reports, RPOC (retained products of conception) is 12 mm. If it is left behind, it can lead to bleeding or cramping or even can cause infection. I would suggest going for a D&C (dilation and curettage).

What can a female do to get regular menstruation despite taking medications for three years?

Hello, Welcome to icliniq.com. The causes of delayed periods are stress, anemia, or thyroid dysfunction. So I suggest you get a complete blood count and a thyroid profile done. You should also get an abdominal ultrasound to rule out PCOD (polycystic ovarian disease), in which multiple cysts form in one or both ovaries. Depo-Provera (Medroxyprogesterone acetate) injection can also delay your periods for up to one year.
