Hello doctor,

My 18-year-old best friend is about 8 weeks pregnant. I found her on the staircase crying as she was in extreme pain and could not move. She said a lot of her pregnancy symptoms like morning sickness and vomiting were gone this morning. I am really worried, please help.

I read your query and can understand your concern.

If your friend is in severe pain, she has to immediately consult the emergency department. From your history, I can only say that maybe her baby is not in good condition. If she does not experience the pregnancy symptoms anymore, it can be an early sign of fetal death. But it is only an assumption from your history.

The first risk factor is her age. She is too young for a pregnancy. Maybe her hormonal support for the baby is inadequate. When this is not corrected by external medications, blood flow to the baby is compromised and the baby finally dies. Once unhealthy, the uterus starts contracting, trying to push the baby out. This is very painful. As for the baby, its surrounding uterine lining and blood are trying to come out of the narrow cervical canal.

She needs to go to the emergency department, can have a shot of a good painkiller to allay the pain, and then have a scan to see the condition of the baby. The doctor has to decide on further action.
