Hello doctor,

At the end of every weekend, I seem to experience severe stomach pain. At first, it feels like I am just hungry. I will still have an appetite for a little bit but if I do not get food soon after I start experiencing slight hunger pains (so I think) the pain gets much worse. I have had ulcers in the past at age 23. Currently, I am 26 years old. The pain that I experience once it gets worse is much like the pain that I experienced when I had ulcers. It will last anywhere from 20 minutes to an hour or more. The longest it has lasted so far is only three hours. After the pain stops I am fine though. I can eat normally and drink liquids normally. Like I said the pain is much like that of the ulcers but not as consistent. This pain has been happening for the past month, I very seldom drink alcohol due to my stomach problems in the past. My eating habits do change from the week to the weekend. I tend to eat a little more food on the weekend but it is usually healthier than what I eat during the week. I avoid fast food if I can, though I do eat a lot of roller food from quick trips and racetracks during the week for convenience while at work. I am not sure what is causing this. I am almost positive it is not ulcers with it only happening once a week like it is but then again I do not know that for sure. Any advice or ideas would be greatly appreciated.

I read your query and understand your concern. Possibility in this case is a duodenal ulcer, this kind of ulcer occurs due to hypersecretion of the acid. We need to evaluate the cause if there is weight loss or intractable pain. But in this case, pain is intermittent but the character of the pain is suggestive of an ulcer in the duodenum. So you should take PPIs for the next two weeks. If there is a response then we should prolong the therapy. H2 blockers may not be effective for a long time, so PPIs (proton pump inhibitors) should be taken. If the pain is not responding then we need to do an endoscopy and if the ulcer is persisting then we should do further investigations after stopping PPIs. Take Tablet Pantoprazole 40 mg once a day for two weeks. Provide details of previously done investigations and bowel habits (constipation or loose stool). Review back with reports and details.
