Hi doctor,

I have been experiencing pain in my right knee for the past 7 months. This began when I started running 6.2 miles on a treadmill daily without proper preparation. The pain has progressively worsened over time. Although I feel pain all around the knee, the main point of pain is just below the knee on the lateral side, specifically in the lateral inferior position. I have been strengthening the muscles around the knee by exercising for about two months now. Sometimes, I also feel a popping sound, which occurs more frequently when I keep my knee in a static position, such as when I am sleeping and make slight movements. The pain on the lateral side is particularly severe when I experience vibrations, such as when riding a bike or traveling on a bus. To manage the pain during these activities, I am using a hinged brace. I have undergone two MRI scans, one 2 months ago, and the other one this week. I am attaching both reports and scans. Please give your opinion.

As you are aware, your recent knee MRI (magnetic resonance imaging) shows changes such as minimal effusion and signal changes in the posterior horn of the medial meniscus, as well as the persistent low-grade partial tear of the anterior cruciate ligament. These findings are not of a serious nature but may be contributing to the discomfort you mentioned. At this moment, it would be beneficial to perform your gym activities under the supervision of an orthopedic or sports medicine doctor. There may be unintentional stress on certain parts of the knee, which might require restriction or modification in the way you perform your exercises.

Thank you for your prompt reply.

Does it heal on its own, or does it require surgery? I am an athlete, and I would like to get back to training as soon as possible. I need a second opinion on the scans. Is it an ACL tear or a sprain?

The ACL (anterior cruciate ligament) injury is mild in nature and does not require any surgery. The terms 'low-grade tear' and 'sprain' are interchangeable and are used for mild ligament injuries where the attachments remain intact. Your recent MRI images show mild hyperintensity and do not necessitate surgical treatment. The signal changes in the posterior horn of the medial meniscus are also mild, and some radiologists might even interpret it as normal. Such issues typically recover on their own. As you are an athlete, there are increased wear and tear mechanisms at work, and therefore your activities might need to be controlled and restricted for a few weeks to allow the existing pain and inflammation to subside.
