Hello doctor,

I have had what I assume is a cystic pimple under my eye for about a week now. The swelling has not reduced; it has only worsened. I have been careful not to touch it, only using Benzoyl peroxide gel and cold compresses. However, this morning, my eye is swollen as well. I must admit that after researching last night, I tried applying Hydrocortisone ointment to help with the swelling. What should I do? Please help.

I have reviewed your query and concerns and examined your picture (attachments removed to protect the patient's identity). Based on your descriptions, it appears that you have solitary or a few cystic acne. I suggest you use a combination of Clindamycin 1%, and Nicotinamide 4% during the day, applying it to the pimples, and Benzoyl peroxide 3.5% GEL at night to your entire face. Continue this regimen for approximately two to three months. In addition to topical treatment, I suggest you take, tablet Azithromycin 500 mg once daily for three consecutive days a week, for a total of four weeks, and capsule Isotretinoin 20 mg once daily, but make sure to consult your general practitioner or a nearby dermatologist before starting this medication.

Before starting the oral medication, get the following tests done: Liver function tests and a lipid profile. If these results are normal, you can begin taking capsule Isotretinoin. However, you must be aware that Isotretinoin is a teratogenic drug and should only be prescribed by a dermatologist. There should be no possibility of pregnancy or conception during the course of treatment and for at least three to six months after discontinuing the medication.
