Hello doctor,

I have been struggling to clear my skin for about three months now. I have never really had bad acne, just a few breakouts here and there, but recently, I broke out and I have been having a hard time dealing with it. I keep breaking out more. I have a combination of oily and dry skin, and I also have hyperpigmentation on my chest and arms from acne. My current medications include Symbicort and Albuterol.

Please advise.

I understand your concern.

Pimples are somewhat hormonal and can be exacerbated by oily skin. I suggest you follow the guidelines below:

  1. Avoid using hair oil and any sticky cream on your face.
  2. Avoid oily food, chocolate, and milk products.
  3. Avoid dust and use sunscreen with SPF (sun protection factor,) above 30.
  4. Do not rub any cream on your face.
  5. Avoid using steroid creams.
  6. Continue treatment for at least one month.
  7. Facewash or soap is not a treatment and is not very useful.
  8. Do not touch acne and avoid pressing it.

For treatment, I suggest the following:

  1. Clindamycin gel: This is both preventive and curative, so I advise patients to apply it all over the face in the morning.
  2. Adapalene gel: I recommend applying it over acne at night.
  3. Tablet Doxycycline 100 mg once a day.

Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

For hyperpigmentation:

  1. Avoid direct sunlight exposure as much as possible. Wear sunglasses, and caps, and cover exposed parts of the body at all times.
  2. Apply sunscreen three times a day, at around 10 o'clock, 12 o'clock, and 3 o'clock, all over the face.
  3. Choose a sunscreen with an SPF of around 50.
  4. Use anti-pigmentation cream on pigmented areas, such as Kojic acid, or Hydroquinone.
  5. If tanning is deep and extensive, consider Glycolic acid peels as the ideal treatment. Typically, six to eight peel sessions are required.
