Hello doctor,

I have acne (inflammation) on the hair follicles on my butt for a long time, which goes on and off, but the marks and blackness of skin have been there forever. The marks are dry and persistent. When a particular one dries, the skin can be easily pulled off and it often has a tiny whitish needle in the middle. It does not pain, but the marks are really bad. I have just used home remedies like aloe vera and coconut oil. Please suggest a lasting solution.

I went through your case. Though photo are bit blurred. Upload a clear one if possible. (attachment removed to protect patient identity). Characteristics you mentioned are spines, dry, and their persistence but again you said they go on and off. I can see some are having small pus. That is important, make it clear if they are always or have any seasonal variation. You did not mention itching, I want to be clear do they itch or not. Do you have a history of allergy in the form of itchy rash? Are these bumps only on buttock area or elsewhere also like elbow, neck, etc? Did they ever make large pus-filled abscess? Do you have any other medical conditions? It is a chronic and rare disease but to be sure I need above history. Then I may advise you a biopsy to confirm the diagnosis.

Do Biopsy for HPE (histopathological examination)

Differential diagnosis

The differential diagnosis are Keratosis pilaris, lichen spinulose, and follicular eczema.

Treatment plan

Apply Ureative20 cream in the morning. Tretinoin 0.05% cream at night.

Preventive measures

Avoid using Moisturizer.

Regarding follow up

Review with history and photo then plan further.

The marks are always there but new ones are red and inflamed with pus, but they dry quickly and become blackish again. It is the same marks which keep recycling from red to black to red again. Sometimes there are no new ones for quite a few days. There is absolutely no itching or pain. These are only in the buttock area. In terms of chronic conditions, I have a problem with shaving or waxing on my legs as it leaves marks on every pore and two to three hairs come out of every pore and does not leave a smooth surface. As such no condition of itchy rashes. Sometimes though very rarely I remember one or two pimples which are painful on the butt, but they are different from these ones, and no pus-filled abscesses as far as I can remember. I have attached more pictures. One picture is of the small whitish needle-like thing that comes, the other is of the dry skin peeled off, and the others are general pictures.

I have seen the pictures. (attachment removed to protect patient identity). This is a hair follicle disorder and the cause is not exactly known. Your history rules out eczema, infection, and lichen spinulosus. Two main possibilities remain are DRIF and keratosis pilaris. They can be differentiated in histopathology only. However, the treatment remains the same. Keep it emoliated with moisturizer like aloevera. Avoid friction as much as possible. Ureative 20 cream and Tretinoin in the morning and evening respectively. Review after four weeks. In severe cases, oral drugs are also given but not needed in your case. Good thing is, it is seen to subside in adulthood. Plan a biopsy by a dermatologist once the lockdown lifts and send these differential diagnoses to a reputed HP lab. The differential diagnoses are Disseminate and recurrent infundibulo-folliculitis (DRIF), Keratosis pilaris rubra, Lichen spinulosis, Follicular lichen planus, Bacterial folliculitis, Pityrosporum folliculitis.
